    Job Description:

    JOB SUMMARY:

    Provides ongoing psychosocial needs assessment of Bennett Cancer Center patients and families  with an emphasis on case work and resource referral. Assists oncology patients and their families with information, linkage, referral  and follow up to internal and external resources including, but not limited to, financial, practical and emotional concerns across the care continuum.   Collaborates with the multidisciplinary team and assists in problem solving.  Knowledgeable of area resources, links patients to resources and coordinates access for patients. Utilizes internal and external resources to assist patients and families in managing the impact of a cancer diagnosis, treatment, survivorship and end of life.  Has a knowledge base of cancer treatments, the impact of cancer on the family,  the psychosocial issues involved, and the ability to educate and support patients and families in this area.  Is involved in the distress screening of all cancer patients and responds appropriately.  Demonstrates the ability to follow up, outreach and coordinate service delivery.  Works closely with staff to meet the psychosocial needs of patients and families.

    -- Completes psychosocial assessment on all patients, develops a plan of care, and makes appropriate referrals. -- Demonstrates knowledge in providing case work services for patients. -- Refers patients/families to internal and external, follows up and coordinates care. -- Documents all patient contact appropriately. -- Involved in the Distress Screening of all BCC patients and responds appropriately. -- Provides crisis intervention services to patients and families as needed. -- Reports to the Manager-Cancer Support Services. -- Collaborates with other staff in providing service to patients and families.

    Required Skills

    Required Experience

    Must hold a CT State LMSW license. Bi-lingual Spanish preferred.
