Follow-Up Protocols: Monitors adherence to structured follow-up intervals, including immediate contact within 24-48 hours of enrollment, weekly follow-ups for the first 30 days, and continued check-ins at 30, 60, 90, 120, and 180 days, adjusting as necessary based on client risk levels.

Missed Appointments and Crisis Response: Ensures follow-up within 8 hours of missed appointments, implementing wellness checks, emergency contact outreach, or other escalations as needed per Zero Suicide engagement standards.

Transition and Discharge Planning: Guides the team in developing transition and discharge plans for clients moving to lower levels of care, coordinating with other providers to ensure smooth, supportive handoffs and continuity of treatment.
