Telligen is working with the Colorado Department of Health Care Policy to support the Long-Term Care Utilization Management Services Program. We are looking for professionals with various areas of expertise (Brain Injury Assessments, CMA Performance Reviews, Post Eligibility Treatment of Income (PETI) Prior Authorizations, Intermediate Care Facility (ICF) Reviews, Hospital Back Up (HBU) Program). What youll be doing: * You will independently conduct face-to-face assessments of persons with disabilities. Collaborate with health care providers and other service providers to build or validate the appropriate care plan. Individuals being assessed reside in the community or in facilities. * You will also document assessment results for the individual member status or negotiate care plan as needed. * You will complete quality assurance reviews and other tasks/projects as assigned such as new assessor training. * You will provide ongoing education and may communicate with health care providers to ensure appropriateness of care. * You may also conduct training of new assessors and quality assurance competency reviews as needed. * You may also complete review decisions regarding services needed for individuals to be successful in the community. What were looking for: * Two or four-year degree or masters degree in a healthcare or related field. * A Licensed Practical Nurse (LPN), Registered Nurse (RN) or Clinical Social Worker/Licensed Professional Counselor (LCSW/LPC) with a current and unrestricted Colorado state license. * Minimum of three years of experience in working with persons with Brain Injuries. * Experience with the Mayo-Portland Adaptability Inventory (MPAI) or other brain assessment tools is highly preferred. * Experience leading Home & Community Based Services (HCBS) or Assessment teams or projects preferred. * Experience in human services and with working with individuals who have disabilities is strongly preferred. * This position is a field-based position where you will travel to cover a geographic area. As business needs and assessment volumes change, additional travel outside of the primary geographic area may be required.
