    GENERAL SUMMARY

    The Patient Family Counselor is responsible for providing counseling and support for patients, families and caregivers. This includes development of goals and objectives and the direct provision of care for patient, family, and caregiver support. Responsible for the most efficient and effective use of human and financial resources in the achievement of patient, family support caregiver support.

    The Patient Family Counselor will consistently demonstrate support of the SRAlab statement of Vision, Mission and Core Values by striving for excellence, contributing to the team efforts and showing respect and compassion for patients and their families, fellow employees, and all others with whom there is contact at or in the interest of the institute.

    The Patient Family Counselor will demonstrate SRAlab Core Attributes: Communication, Accountability, Flexibility/Adaptability, Judgment/Problem Solving, Customer Service and SRAlab Values (Hope, Compassion, Discovery, Collaboration, & Commitment to Excellence) while fulfilling job duties.

    PRINCIPAL RESPONSIBILITIES

    THE PATIENT FAMILY COUNSELOR WILL:

    1. Communicate findings to physicians, nurses, allied health and extended interdisciplinary team.
    2. Carry out social evaluations including, family dynamics, caregiving abilities, communication patterns, high risk for suicide, neglect or abuse and plans based on evaluation findings.
    3. Counsel patients and family/caregivers as needed in relationship to others and other identified coping difficulties.
    4. Provide crisis intervention when necessary.
    5. Assess for and educate interdisciplinary group on any special needs related to the culture of the patient/family/caregivers, learning style, learning limitations, communication style, roles within the family and any special traditions or taboos.
    6. Maintain clinical records on patient/family/caregiver.
    7. Assist with advanced directives, transitions to next level of care.
    8. Serves on various ad hoc and standing committees as requested.
    9. Responsible for providing direct patient care to a consistent caseload of patients assigned typically by Innovation Center.
    10. Perform all other duties that may be assigned in the best interest of SRAlab.

    REPORTING RELATIONSHIPS

    1. Reports directly to the Director of Allied Health
    2. Supervises one to two students per year and 4 – 5 weekly volunteers.

    KNOWLEDGE, SKILLS & ABILITIES REQUIRED

    1. Knowledge of patient family care giver support, including the necessary knowledge, experience and clinical skills at a level acquired through completions of a Master’s degree.
    2. Licensed Social Worker, Licensed Clinical Social Worker or Licensed Professional Counselor preferred.
    3. Demonstrated ability to manage a patient family caregiver support caseload typically acquired through minimally one to three years of progressively more responsible experience in a hospital rehabilitation center for individuals with physically disability.
    4. Analytical ability necessary to solve complex administrative and clinical problems. Ability to evaluate needs of patient populations and develop programs to meet those needs.
    5. Interpersonal skills necessary to motivate and provide direction as well as represent the SRAlab outside of the Institute to community groups and officials of health and welfare agencies. Ability to communicate effectively with all levels of management within the Institute.
