    GENERAL SUMMARY:

    The APP (NP, PA-C) will work in collaboration with the attending medical staff to provide individualized patient care through the assessment and medical management of a broad spectrum of diagnoses. Additionally, s/he promotes the highest quality of patient care utilizing research and evidence based practice to guide decision-making.

    The APP is responsible for providing high quality care to inpatients and functions as a member of the medical team. Duties include performing all tasks involved with admission and discharge processes, including complete admission H&Ps, ordering and prescribing medications, medication reconciliation, ordering diagnostic and therapeutic tests and procedures, developing comprehensive medical/rehabilitation assessments and plans of care and discharge summaries.

    PRINCIPAL RESPONSIBILITIES: 

    CLINICAL

    Elicits an accurate history and physical examination which may be focused or comprehensive to assist the attending physician in establishing a diagnosis, plans and implements interventions, prescribes medications, therapies and diagnostic tests according to established guidelines, and evaluates patient responses to therapies under the direction of a physician. Demonstrates advanced critical thinking in the care and management of patients.

    1. Assembling a complete medical database including health history, history of chief complaint, and physical examination of patients on admission/readmission.
    2. Under the direction of and in consultation/collaboration with the attending physician, orders medical consultations, therapies, and interventions including, but not limited to, medications, diagnostic tests, respiratory care to meet patient needs and ensure continuity of care.
    3. Provide education to patients and families.
    4. Collaborate effectively with the nurses, therapists and manager to implement and evaluate comprehensive patient care delivery and establish best practices in clinical care.
    5. Potentially manage acute care issues that may arise on inpatients under the supervision of the attending physicians.

     REPORTING RELATIONSHIPS 

    1. Reports to Medical Director.
    2. Reports to designated Physician for clinical supervision

      KNOWLEDGE, SKILLS & ABILITIES REQUIRED   

    NURSE PRACTITIONER:

    1. Master’s Degree in an approved Nurse Practitioner program.
    2. Certification by the American Nurse’s Credentialing Center as a nurse practitioner.
    3. Licensure as a professional nurse and APN in the State of Illinois.
    4. Minimum of three years of experience as a professional nurse and/or Nurse Practitioner required.
    5. Requires interpersonal skills necessary to communicate effectively with patients, patient families, referring parties, Institute executives and treatment team members.
    6. Require ability to demonstrate sound medical knowledge and leadership capability.
    7. Advanced cardiac life support certificate preferred.

    PHYSICIAN ASSISTANT:

    1. Graduate of an approved Physician Assistant program.
    2. Licensure as a Physician Assistant in the State of Illinois.
    3. Current certification from National Certification Commission for Physician Assistants (NCCPA)
    4. Minimum of three years of experience in the health care field.
    5. Requires interpersonal skills necessary to communicate effectively with patients, patient families, referring parties, Institute executives and treatment team members.
    6. Requires ability to demonstrate sound medical knowledge and leadership capability.
    7. Advanced cardiac life support certificate preferred.

     WORKING CONDITIONS   

    1. Normal office environment with little or no exposure to dust or extreme temperature.
    2. Exposure to moderately adverse working conditions in performing certain patient care activities.
    3. Occasional exposure to infectious diseases when working with patients.
