  • Responsibilities

    The Transplant Coordinator delivers and accurately documents direct, age-specific care to a patient or group of patients based on medical orders and an integrated, multidisciplinary plan of care. This job safely administers prescribed medications, identifying incompatibilities and/or interactions. This job manifests a high level of expertise in the assessment, coordination and care of the complex patients, families, or communities to actual or potential health problems; prevention of illness and injury; maintenance of wellness; and provision of comfort.

    The Transplant Coordinator demonstrates strong clinical experiences in patient care. The position demonstrates a high level of expertise in the assessment, diagnosis, and treatment of the complex patients and their responses to actual or potential health problems. The position sssists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort. The position also provides direction, education, and leadership.

    Licensure, Certification, or Registration Requirements for Hire:
    Current RN licensure in Virginia or eligible

    Licensure, Certification, or Registration Requirements for continued employment:
    Current RN licensure in Virginia AHA BLS HCP Certification or equivalent

    Experience REQUIRED:
    Minimum of three (3) years of nursing experience in transplant, critical care or medical-surgical nursing
    Two (2) years of progressive leadership experience
    Demonstrated clinical expertise for population served with a minimum of one (1) year in specialty area

    Experience PREFERRED:
    Five (5) years of clinical experience in the transplant field Academic Healthcare experience
    Advanced Practice Nursing Role or Clinical Leadership experience

    Education/training REQUIRED:
    Baccalaureate Degree in Nursing from an accredited School of Nursing

    Education/training PREFERRED:
    Masters Degree in Nursing Certification in Transplantation

    Independent action(s) required:
    Follows documented physician/licensed independent provider orders, within the boundaries of the Nurse Practice Act and established standards, policies and practice guidelines.
    Organizes and plans work with input from the patient/family with specific outcomes and demonstrated use of sound clinical judgment and resources.
    All day-to-day operations of the role are handled independently with minimal assistance.
    Advice and guidance are sought from the Manager or Director on an as needed situational basis and when there is a major impact on activity outside the unit.
    Actions that are taken without prior approval include daily operational functions and leadership of staff.
