    Position Summary:
    We have an exciting opportunity to join our team as a Psychiatric Social Worker.

    In this role, the successful candidate The Social Worker is responsible for providing high quality patient care, displaying good skills in the assessment, diagnosis, treatment planning, group treatment, individual treatment, family treatment, and discharge planning of patients presenting with psychiatric and/or co-occurring Substance Use Disorders. Is an integral member of the multidisciplinary team. Conducts insurance authorizations and reviews and communicates results with Team members. Functions as a Clinical resource for the Department and other Hospital Departments. Adheres to all NYS Office of Mental Health regulations

    Job Responsibilities:

    Provision of Patient Care:

    * Consistently ensures patient dignity and respects patient values and operates within ethical standards.
    * Demonstrates ability to assess and reassess the needs of the patient as per regulatory guidelines.
    * Accurately assesses the needs of assigned patients within the parameters of length of stay and discharge criteria.
    * Demonstrates the ability to communicate needs of patients with treatment team.
    * Completes psychosocial evaluations on assigned patients in a timely fashion and displays good interviewing, diagnostic, and report-writing skills, as needed.
    * Conducts therapies in accordance with patient diagnosis and needs.
    * Provides efficient discharge planning throughout treatment process.
    * Conducts individual, family, and group psychotherapy sessions, displaying good theoretical knowledge of psychopathology and good treatment technique and provides ongoing education to patient and family as needed.
    * Engages in timely, efficient discharge planning within 24 hours of admission. Works in close conjunction with patient, family, nursing staff, and attending physician. Shows good knowledge of referral/placement process and available community resources.
    * Conducts educational group sessions, displaying good theoretical knowledge of subject matter and effective communication skills in presenting material.
    * Communicates effectively with families and agencies, conveying a positive, professional image.
    

    Meets Expected Documentation Requirements:

    * Ensures confidentiality of all patient information encountered by obtaining required consents for release of information.
    * Completes progress notes, treatment plans, and discharge plans in a timely and high-quality fashion and according to department standards.
    * Adequately documents all patient encounters in chart on a consistent basis and takes responsibility for chart organization for easy access to information.
    * Completes individualized treatment plan within time parameters for each patient
    * Conducts and documents ongoing insurance certifications as indicated.
    * Appropriate consents and releases of information collected from patients.
    * Makes Adequate Contribution to Overall functioning of the Department 
    * Actively involved in measuring patient performance and communicates results and trends to supervisor.
    * Actively participates as an interdisciplinary team member on behalf of the patient.
    * Provides feedback to supervisor on ongoing basis in regard to concerns, improvements, changes, etc.
    * Identifies areas needing improvement and utilizes the facility performance improvement process. Actively participates in department processes as required.
    * Utilizes feedback regarding performance and uses supervision from Clinical Director, Management Team, to improve performance.
    * Exhibits willingness to master new skills and accepts change as necessary within the department.
    * Participates in Quality Improvement activities as needed to continually improve their department’s performance.
    * Regularly acts as a resource for information on Behavioral Health for other departments, customer, and other agencies as necessary for patient care and community relations.
    * Continually strives to make productive use of time through careful coordination of daily tasks, setting priorities, and reducing non-essential interruptions in order to complete the job during the allocated time.
    * Interacts and communicates with others in a way that promotes a positive and cooperative work environment.
    * Demonstrates initiative and enthusiasm in performing job duties on a daily basis.
    * Shows willingness to adjust personal schedule in case of workload fluctuations.
    * Consistently maintains composure and professionalism during difficult situations.
    * Demonstrates understanding of concepts of excellence in customer service and conducts duties in positive, customer-friendly manner to all customers of department, particularly patients, visitors, physicians, and other employees.
    * Voluntarily helps other Behavioral Health staff as necessary, to promote teamwork.
    * Maintains a neat, clean and organized work environment.
    * Attends all meetings as scheduled.
    * Performs other duties as assigned by Behavioral Health Administrative Team in a timely manner.
    

    Provision of Interdepartmental Collaboration:

    * When patient care is not compromised, assists the Emergency Department or other departments in the hospital with patients in acute behavioral health crisis as requested by staff in respective departments. Accepts assignment to other areas of the Behavioral Health department and other departments as requested by Behavioral Health Administrative Team or Nursing Supervision.
    * Works effectively with other Hospital departments.
    * Shows willingness to be cross-trained to support other areas of the department as needed.
    * Possesses Core Skills to Work Effectively in a Behavioral Health Setting.

    Minimum Qualifications:
    To qualify you must have a Masters degree in Social Work.
    Current New York State license to practice social work.
    Knowledgeable in federal, state and local regulations.
    Knowledge of outside health-related and community resources for Behavioral Health patients.
    Able to work effectively in a chaotic crisis environment.
    Two years experience working with psychiatric patients preferred.
    Knowledge of non-violent crisis intervention technique or ability to learn mandatory Secure Training.
    Knowledge of Restraint policy.

    Long Island Community Hospital provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $55.13 - $55.13 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.   • Qualifications
    • Strong communication skills with a solid understanding of advertising principles, design best practices, typography, web and media trends, and marketing strategy.
    • Understanding of web trends and basic HTML/CSS skills for active participation in web design and web graphic creation.
    • Proficiency with Adobe Creative Suite and Mac platform.
    • Proficiency with Microsoft Office Suite, Adobe Premiere, and/or Final Cut Pro.
    • Ability to multitask in a deadline-driven environment and work well within a multidisciplinary team.
    • Expertise with both raster and vector-based graphics, including the creation, retouching, and editing of photographic and illustrative imagery.
