Job Summary:
The Care Coordinator is responsible for organizing and managing all aspects of a patient’s care at TCPC. They will support the clinic and provider care teams by delivering administrative and patient care services. Their goal is to take initiative to improve the patient’s care and experience. Essential functions of the Care Coordinator position are:
- Direct contact with patients, both in-person and via telephone
- Communicate with the healthcare team about a patient’s treatment plan
- Help patients complete paperwork
- Make phone calls
- Coordinate with other departments and outside facilities
- Schedule procedures
- Pre-procedure screening and education
- Assist with prescription prior authorization and completion of forms
- Coordinate in-office medication dispensing and/or mail-order refills
- Coordinate patient appointments, including diagnostic tests, physical therapy, and pain psychology, in accordance with the patient’s treatment plan.
- Assist clinical team with patient tracking.
- Updates patient information in EHR including medications, allergies, primary care physician, pharmacy, etc.
- Enroll patients in the patient portal.
- Coordinate the transmission and acquisition of necessary patient records.
- Manage all anticoagulant hold requests. Facilitate timely completion of the request. Contact patient, prescriber, and pharmacy as needed.
- Cross train into the medical assistant role and prescription prior authorizations
Supervisory Responsibility:
- This position has no supervisory responsibilities.
Qualifications:
- Minimum one year of strong customer service with an exceptional ability to listen and communicate with others.
- High school diploma/GED plus one year medical clinic experience
OR
- Associates or Bachelor’s degree

While performing the duties of this job, the employee is occasionally required to walk; sit; use hands to finger, grasp, handle, objects, tools, or controls; reach with hands and arms; stoop, kneel, or crouch; talk or hear. The employee must occasionally lift or move up to 20 pounds. Employee will be walking or standing for 60% of the time in this role and will be sitting for 40% of the time in this role. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.
Compensation & Work Schedule:
- Full-time: 40 hours/week.
- Non-Exempt.
- Day Shift Monday – Friday.
- No evenings or weekends.
