    The Community Health Nurse is responsible for identifying healthcare obstacles in the community, provide health education and promotes healthy habits; to encourage community members to take charge of their own health. Develops new models of healthcare delivery; reducing barriers to care through dynamic partnerships with a focus on health maintenance and illness prevention.

     

    This position is also responsible for providing medical/health expertise to the Maternal and Child Health Coalition’s (MCHC’s) Kansas City Healthy Start (KCHS) program. This role will work to identify women at high risk for maternal mortality and infant mortality and those with gaps in care in order to assist in navigating them to the care needed to deliver a healthy baby. This specific role is grant funded, should funding for the grant cease, this position will be terminated.

    Required Skills

    • Bachelor of Science in Nursing
    • RN licensed in the state of Missouri
    • Excellent interpersonal, oral, and written communication skills; mastery of public speaking and presentation skills
    • Knowledge of and ability to effectively utilize coaching, motivational interviewing, and solution-oriented theory
    • Demonstrated ability to set priorities and work independently
    • Knowledge of community or public health nursing and practices
    • Has a personal, demonstrated passion for health and wellness and walks the walk by developing personal wellness plans and pursing mastery of their own personal health and well-being
    • Basic computer skills
    • Possess and maintains a valid drivers license and appropriate automobile insurance
    • Ability to work evening and weekends

    Required Experience

    • 2-5 years experience working in Women's Health or Labor and Delivery care
