Provides social work services in acute, ambulatory, rehabilitative, and long-term care settings. Services are provided for patient''s/residents and families receiving acute, chronic or long-term care. Services include high risk identification, psychosocial assessment, pre-admission screening, admitting services for long-term care patients, discharge planning, emotional support, information and referral, protective services, and patient education. functions as a member of an interdisciplinary team for patient care. Assists in program development and evaluation. Works under regular supervision and direction of a MSW.

    Required Skills

    • Bachelor''s of Social Work (BSW) from a school accredited by the Council on Social Work Education (CSWE).
    • Knowledge of the theory and practice of social work and skills commensurate with training at the BSW level.
    • Ability to function effectively as part of a multidisciplinary team.
    • Good verbal and written communication skills.
    • Must maintain CPR certification for continued employment. Preferred Qualifications
    • Previous social work experience in health or long-term care setting.
