SM PROHEALTH PHYSICIANS HAS AN IMMEDIATE OPENING FOR A FRIENDLY, CUSTOMER-FOCUSED AND DETAIL-ORIENTED PHARMACY TECHNICIAN TO JOIN OUR WELLHOP CARE TEAM. THIS FULL-TIME POSITION WILL BE BASED FARMINGTON, CT AND WILL BE AN 8 AM TO 5 PM OPPORTUNITY. THE PHARMACY TECHNICIAN WILL WORK CLOSELY WITH THE CLINICAL PHARMACIST(S) TO COLLABORATIVELY PRODUCE PHARMACIST CONSULTATIONS RELATING TO DRUG THERAPY FOR PATIENTS WITH DIABETES. A SUCCESSFUL CANDIDATE WILL POSSESS A ACTIVE PHARMACY TECHNICIAN LICENSURE WITH STRONG CLINICAL KNOWLEDGE, SKILLS, AND EXPERIENCE. PRIMARY RESPONSIBILITIES:

• Working closely with the clinical pharmacist(s) to collaboratively produce pharmacist consultations relating to drug therapy for persons with diabetes
• Understand and implement strategies to engage primary care providers (PCPs), specialists, managerial & clerical staff, and dispensing pharmacies in collaborative care of patients
• Transcribe data from the electronic health record (EHR), including lab values and medication history, to patient case notes
• Attend and actively participate in daily "huddles" with the Wellhop Diabetes Care team to continuously improve work processes and patient care
• Communicate clearly, concisely, and professionally to collaborators in verbal and written format
• Maintain and organize patient records and process summaries
• Navigate pharmacy benefit manager (payer) specifics both on-line and by phone
• Assess opportunities to reach patients for initial and follow-up consults based on the date of project enrollment, office visits, and previous contacts with the Wellhop team

• Pharmacy Technician certification active licensure through the State of Connecticut
• Current and unrestricted Pharmacy Technician licensure through the State of Connecticut
• 3+ years of experience working in a pharmacy as a Pharmacy Technician
• Strong Microsoft Word, PowerPoint, and Excel experience and the ability to type and navigate in a Windows environment
• Must be flexible and available to work the designated shift

PREFERRED SKILLS:

• Very personable
• Prior EHR experience
