This is the place to do YOUR LIFE'S BEST WORK.(SM) The PRACTICE PERFORMANCE CONSULTANT RN is responsible for program implementation and provider performance management which is tracked by designated provider levers, including referrals to high performing specialists and in-network labs. The person in this role is expected to work directly with care providers to build relationships, ensure effective education and reporting, proactively identify performance improvement opportunities through analysis and discussion with subject matter experts; and influence provider behavior to achieve needed results. Work is primarily performed at physician practices on a daily basis. If you are located in Houston, Texas, you will have the flexibility to telecommute* as you take on some tough challenges. PRIMARY RESPONSIBILITIES:

    • Functioning independently, travel across assigned territory to meet with providers to discuss designated levers and associated tools and reporting in order to improve overall provider performance and encourage cost-savings
    • Execute applicable provider performance improvement plan
    • Establish positive, long-term, consultative relationships with physicians, medical groups, IPAs and ACOs
    • Develop comprehensive, provider-specific plans to increase their performance and improve their outcomes
    • Provide ongoing strategic recommendations, training and coaching to provider groups on program implementation and barrier resolution
    • Act as lead to pull necessary internal resources together in order to provide appropriate, effective provider education, coaching and consultation. Training will use of plan tools, reports and systems
    • Provide reporting to health plan leadership on progress of overall performance, lever migration, and potential barriers in the market
    • Facilitate/lead monthly or quarterly meetings, as required by plan leader, including report and material preparation
    • Provide suggestions and feedback to Optum and health plan
    • Work collaboratively with health plan market leads to make providers aware of network specific resources designed to assist providers in cost savings and proper utilization of care partners
    • Provides care coordination/case management through physician practices for members to improve clinical quality and clinical documentation. No direct patient care
    • May interact with patients via telephone; Schedule appointments, Follow up calls to assess understanding of services, answer questions and ascertain that additional procedures have been completed
    • Includes up to 75% local travel

    • Active, unrestricted RN license in the state of Texas
    • 3+ years clinical experience in a hospital, acute care, home health/hospice, direct care, case management or med adherence
    • Reliable transportation to travel to provider groups or other locations within service delivery area
    • Ability to travel up to 75% within the assigned metropolitan Houston territory
    • Microsoft Office specialist with exceptional analytical and data representation expertise; Advanced Excel, Outlook, and PowerPoint skills
    • Access to a home office with access to high speed cable/DSL internet

    PREFERRED QUALIFICATIONS:

    • Bachelor's Degree
    • Experience with navigating and analyzing reports in Microsoft Excel
    • Prior Physician Office experience
    • Home care/field based case management
    • Strong problem-solving skills
    • Strong communication and presentation skills
    • Strong relationship building skills with clinical and non-clinical personnel
    • Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs
