This is an opportunity to learn the future of ambulatory care pharmacy services. This position allows interns to work in three unique areas; Pharmacy Admissions and Transitions of Care Services, Atrium Outpatient Pharmacy and the Medication Access and Renewal Center (MARC). Based on individual performance, the successful intern will have the opportunity to work closely with pharmacists and nurses to provide clinical services in these areas. There will be opportunities to work on projects to complement your academic learning and increase your clinical practice experience. Job Duties: Pharmacy Admissions and Transitions of Care Services · Perform medication reconciliation on newly admitted patients in order to reduce inpatient medication errors · Address discharge medication barriers · Assist with discharge medication reconciliation · Conduct discharge medication counseling or counseling about the patient's disease state · Provide a list of the patient's discharge medications to their community pharmacy · Follow up with the patient telephonically 2 days after discharge Atrium Outpatient Pharmacy · Coordinate the fulfillment and bedside delivery of discharge prescriptions by collaborating with the patient, family members and their healthcare team · Conduct discharge medication counseling Medication Access and Renewal Center (MARC) · Complete prior authorizations for specialty medications by clinically reviewing provider office visit notes, pertinent labs and medication history · Identify appropriate financial assistance options so patients can afford their medications · Conduct periodic clinical assessments of patients taking specialty medications in order to assess medication adherence, occurrence of adverse effects and therapeutic effectiveness. · Assist pharmacists in writing appeal letters and/or identifying therapeutic alternatives for medications denied by insurance · Perform thorough chart reviews to evaluate the appropriateness of refill renewal requests · Authorize refill renewal request using an evidence-based drug therapy management protocol General · Participate in assigned education and training programs · Design and present educational programs for pharmacy staff, other medical personnel and patients · Participates in the UCH Ambulatory Care Pharmacy Services quality management programs · Complete administrative and clinical projects · Work a minimum of 12 hours/week throughout the year, including rotating through one weekend day shift every 2-3 weekends Requirements: · Currently enrolled in an accredited school of pharmacy · Active Colorado Intern License · On-the-job training will be required Preferred: · First or second year pharmacy students preferred · Previous Pharmacy Technician experience preferred The University of Colorado Health system is home to five hospitals, three of which hold Magnet designations by the American Nurses Credentialing Center, as well as multiple Nightingale awards for excellence in nursing.
