    We are seeking experienced Social Workers to join our extraordinary team!

    As a Social Worker II you will:

    • In collaboration with a multidisciplinary team, the social worker is responsible for completing intake assessments, assisting with treatment planning, monitoring patient progress, facilitating patient and family meetings, and coordinating and implementing discharge services for patients in the assigned client area(s). 
    • May also provide psychotherapy utilizing brief therapy models incorporating crisis stabilization and cognitive behavioral techniques. 
    • Participate in health quality improvement and evaluation processes.

  • Qualifications

    Qualifications

    QUALIFICATIONS:

    • MSW from a school of social work accredited by the Council of Social Work Education required. 
    • Maryland licensure at the LCSW or LCSW-C required. 
    • Experience working with individuals with acute, chronic, and social needs required. 
    • Acute care social work experience preferred. 
    • Knowledge of Care Management preferred.  Understanding of insurance eligibility and coverage requirements.

     

    WHAT YOU WILL NEED TO BE SUCCESSFUL:

    Position requires accountability for planning, organizing, and directing discharge and aftercare services that support patient self-management, adherence to treatment plans, and reduce unnecessary utilization.
