    ********General Summary
    Under general supervision, responsible for the overall, daily operations of the residence. Based on a strengths model and using identified counseling and communication skills, engages members to develop and maintain a supportive, therapeutic, safe, home-like atmosphere which is sensitive to individual cultural and functional differences.

    Principal Responsibilities and Tasks
    **The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.

    1. Trains members on daily independent living skills by developing effective rapport as reflected in their Individual Rehabilitation Plan (IRP),
    2. Provides input and makes recommendations to the team on needs, skills, and preferences of members to ensure the Rehab process is to be successful.
    3. Identifies social, environmental and health care needs and meets these needs by helping members develop daily living skills, and monitors Health Homes Goals as established in the Health Home Plan.
    4. Instructs, supervises and participates in performing daily living skills/tasks with members, such as, cleaning, self-care/grooming, preparing meals, planning household budgets, food shopping, interpersonal relationships and clothing purchases. Supports members in daily routines and teaches relationship development and leisure skills.
    5. Conducts and documents regular safety drills. Instructs and assists members to perform routine household maintenance.
    6. Holds regularly scheduled house meetings and counseling groups with members. Communicates household activities to the team.
    7. Liaison to the neighborhood for members. Develops individualized natural supports and resources.
    8. Completes daily billing sheets, house logs, maintenance request forms, medication documentation, progress towards IRP/, and incident reports as they occur.

    Education and Experience**

    1. High School Diploma or equivalent (GED) is required. Bachelor’s degree in a Human Services related field is preferred. Bachelor’s degree may substitute for the experience requirement on a year for year basis.
    2. Minimum of one (1) year experience in mental health services, or equivalent health services experience, is required.
    3. A current, valid, MD state driver’s license is required. Not more than two points on the driving record per year permitted.

    Knowledge, Skills and Abilities**

    1. Knowledge of basic home routines and maintenance tasks (i.e. change a light bulb, plunge a toilet, change a fuse, prepare nutritious meals, etc.) is required.
    2. Effective oral and written communication and counseling skills (i.e. active listening, building rapport, respecting boundaries, etc.) are necessary in order to establish and maintain effective working relationships with members, staff members and the general public.
    3. Actively participates in team meetings to ensure efficient and effective service provision. Attends training to remain up to date on current best practices.
    4. Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient populations(s) served. Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient’s requirements relative to his or her population-specific and age specific needs.

    Patient Safety
    **Ensures patient safety in the performance of job functions and through participation in hospital, department or unit patient safety initiatives.

    1. Takes action to correct observed risks to patient safety.
    2. Reports adverse events and near misses to appropriate management authority.
    3. Identifies possible risks in processes, procedures, devices and communicates the same to those in charge.
