ADDRESS: 5910 W. Division Chicago, IL 60651 SCHEDULE: Hours vary RESPONSIBILITIES:

• Provides outreach to engage high risk youth in services 
  • Meet with parents and guardians to assist engagement
• Provides counseling, employment, educational and life skills training
• Coordinates with other YOS services to engages youth and families in appropriate services
• Provides transportation to youth as necessary
• Maintains contact or establishes relationships with referral sources beneficial to the client and/or agency
• Makes referrals to appropriate programs and/or services both internally and externally and formulates necessary follow-up in a timely manner
  • Refer youth to services as needed
• Works with youth and other collaterals to develop service plans that assist youth in addressing challenges
• Documents all interactions in designated electronic record
• Participates in team/individual supervision and meetings as necessary
• Collaborates with co-workers and participates in internal agency programs to provide the most comprehensive client services

QUALIFICATIONS: 

• BA in Social Work, Counseling, or Psychology is required
• Experience working with at-risk youth and families
• Experience with outreach programs
• A valid driver's license
• Reliable transportation (proof of insurance is required)
