RESPONSIBILITIES:
- Provides outreach to engage high risk youth in services
- Meet with parents and guardians to assist engagement
- Provides counseling, employment, educational and life skills training
- Coordinates with other YOS services to engages youth and families in appropriate services
- Provides transportation to youth as necessary
- Maintains contact or establishes relationships with referral sources beneficial to the client and/or agency
- Makes referrals to appropriate programs and/or services both internally and externally and formulates necessary follow-up in a timely manner
- Refer youth to services as needed
- Works with youth and other collaterals to develop service plans that assist youth in addressing challenges
- Documents all interactions in designated electronic record
- Participates in team/individual supervision and meetings as necessary
- Collaborates with co-workers and participates in internal agency programs to provide the most comprehensive client services
QUALIFICATIONS:
- BA in Social Work, Counseling, or Psychology is required
- Experience working with at-risk youth and families
- Experience with outreach programs
- A valid driver's license
- Reliable transportation (proof of insurance is required)
