About this Role
This role ministers to and serves others by providing compassionate, Christ-centered care that supports guests on their journey toward stability, healing, and self-sufficiency. Through biblical guidance, advocacy, case management, and connection to community resources, the Case Worker helps guests overcome barriers and experience lasting life change. What this role looks like
Ability to provide Biblical guidance through life change.
Assesses needs and coordinates services with internal teams and community agencies.
Supports Guests' work towards achieving their self-identified goals for positive, life-changing outcomes.
Collaborates with fellow case workers as client needs may also require their services.
Manages a case load of individual clients and maintains ongoing client files and daily case notes.
Maintains accurate files on all services and outcome results provided to and from clients.
Approves and dismisses guests as appropriate.
Tracks client outcomes and exits to prepare reports for ongoing case management.
As part of the Guest Services team, assists with daily dayroom operations.
Network with community agencies to establish and build positive working relationships.
Stay up to date with housing programs and adjust to processes as well as stay informed with restoration House and Greer Street about available housing options.
Conduct intake assessments to determine client needs related to Social Security and financial assistance programs.
Engage with the healthcare system and guests to access, plan, and assist the common goal of stabilization of health.
Provide client advocacy when needed to potential service providers.
Educate and equip guests with tools necessary to enter appropriate housing.
Prepares monthly reports to Guest services Manager, as required.
Attends community meetings as directed by Guest Services Manager.
What we're looking for
- Excellent oral and written communication Skills
- Proficient in Microsoft Office Suite (Excel, Word and Outlook).
- Familiar with community social service providers and knowledge of substance abuse and recovery required.
- Valid Oregon Driver's license and current auto insurance.
- Must agree with UGM's Statement of Faith and Core values.
