CCS Service Facilitator Racine County 16 reviews - Racine, WI 53404 $19.53 - $27.47 an hour Basic Function To assess children and adults as requested for admission to the CCS program, coordinate services between providers and to provide outreach services designed to increase access to CCS services. Essential Duties * Screen referrals for program eligibility; meets with referred participants and their families. * Complete assessments of participant and their families using required CCS assessment tools. * Provide support and assistance and facilitate participants in development of self-advocacy skills, formulate recovery focused client-centered treatment plans * Maintain a treatment oriented environment and assure consistency in the identification of recovery or treatment objectives. * Coordinate and facilitate utilization of various community resources and providers. * Assist participant to identify and build a recovery team including the participant's family and/or other individuals who support the participant's recovery, when appropriate. * Adhere to agency policies and procedures while carrying out daily responsibilities. * Accurately document and maintain case file as required by agency policy, government regulation and applicable local, state and federal statutes. * Participate in staff meetings and in-services trainings, as required. * Other job related duties may be necessary to carry out the responsibilities of the position. Work Relationship and Scope CCS Clinical Coordinator. Works with other case managers, registered nurse, consulting psychiatrist, pharmacy, director of quality assurance, consumers and managers. Has outside contact with community agencies, consumer family members, and various hospital units. Qualifications * Bachelors Degree in a human services field (child development, educational psychology, educational rehabilitation, counseling, guidance and counseling, psychology, social welfare, social work or sociology). Masters of Social Work or Counseling preferred. * One year full time post experience in the field of child welfare, or juvenile justice (experience does not include internships, school or volunteer experiences). * Experience with treatment programs involving adults with mental illness, children with emotional disturbance, and/or adults and/or youth with substance use disorder. * Social work experience must include experience in at least 4 of the following activities: assessment, * Experience in coordinating services between multiple systems, e.g., health, education, social services, mental health. * Prompt and regular attendance. Knowledge Skills & Abilities * Must have ability to communicate effectively orally and in writing, and interact effectively with clients and other health care professionals.
