    SSVF (Supportive Services for Veteran Families) Coordinator Wayne Metropolitan Community Action Agency 18 reviews - Westland, MI # of Open Positions:1 Salary Grade: 3 FLSA: Non-Exempt Employment Type: Full-Time/Regular Benefits: Yes Hours per Week: 40 Work Days: Monday-Friday Reporting to: Supportive Housing and Homeless Services Manager Recommended Start Date: ASAP Scope: Responsible for the provision of comprehensive Housing services, utilizing the Housing First Model, Veterans and their families who are experiencing homelessness or who are marginally housed, to include, intake/assessment, case planning, and service coordination. Essential Functions * Work closely with community and government agencies (social services, shelters, faith and community based organizations, jails, VA facilities, landlords, etc.) to identify Veteran families who are at risk of or are experiencing homelessness * Refer ineligible families to alternate resources/supports as available in the community. * Work closely with clients to develop and implement a client-centered, individualized case plan with a focus on achievement of housing stability. * Coordinate with VA services, Veteran programs, and other community resources to assist clients in seeking income, employment and/or other needed supports. * Link clients to internal in the form of direct client assistance payments to third party (e.g., landlords). * Gather information on potential resources for clients in the larger community * Responsible for training all new hires for the SSVF program on program guidelines and resources. * Acts as the data quality specialist for all SSVF staff and sub-grantees * Maintain clear and precise data and case notes. * Actively participate in weekly CoC Partnership Meetings. * Participate in Out Wayne County Continuum of Care and any necessary sub-committees. * Stay informed of best practices for serving Veterans and sub-populations of Veterans. * Participate in VA initiatives based on program need. * Coordinate the SSVF program for the optimal delivery of services. * Provide training and expertise to sub-contractors in relation to program guidelines to ensure compliance.

    • First point of contact for any issues, questions or complaints regarding the SSVF program before being escalated to Manager. * Plan quarterly peer file reviews for all direct and sub-grantees of the WMCAA SSVF program. * Provide input to outreach and housing databases. * Attend provider meetings in neighboring communities to ensure unduplicated services. * Provide coordination for VASH transition. * Maintain and demonstrate compassion toward program candidates and participants while operating within the policies of WMCAA and the SSVF grant. * Maintain and demonstrate SSVF Program but for objective which ensures Temporary Financial Assistance is provided as a last resort while utilizing available resources first. * Build relationships with the community at large to advertise services and to strengthen partnerships. * Coordinates special projects as identified by CPO Basic Needs, SHHS Director and SHHS Manager. * Maintain a positive attitude towards assigned work, clients, and staff, and a willingness to work with others. * Perform work in a professional manner that upholds Wayne Metros policies and procedures. * Other duties as assigned. Physical Requirements and Work Environment Clerical: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. This job operates in a professional office environment with a usually quiet noise level. This is largely a sedentary role and the employee must occasionally lift and/or move up to 25 pounds. While performing the duties of this job, the employee is regularly required to sit, frequently use hands and to talk or hear. Minimum Qualifications: * Bachelor Degree in Human Services with a minimum of three years of experience in a related field OR a combination of education and experience that is similarly equivalent. * Knowledge of Wayne County demographics, the needs of people experiencing homelessness or who are at risk of losing housing in Wayne County, and services available to meet those needs. * HMIS experience a plus. * Veteran experience a plus. Minimum Competencies: * Working knowledge of basic internet applications and programs including Google technology and Microsoft Office Word and Excel. The ability to learn and utilize the necessary programs for this position. * Ability to communicate effectively both orally and in written form. * Good interpersonal and professional skills including but not limited to professional appearance, tact, punctuality, and dependability. * Ability to provide services for those persons facing economic, social, health, age, and educational barriers. * Ability to work independently or in a team environment (as deemed necessary). Licensing and Other Requirements: * Valid drivers license and reliable transportation. Travel Requirements: * Ability to travel to various sites. Work Hours: Monday through Friday, 8:30 am 5:00 pm.
