  • Responsibilities

    The Transition Coordinator is a direct service provider working with youth and young adults to provide pre-employment transition services as well as some job placement and coaching.  This position serves students and families in Utah County.  Responsibilities include but are not limited to the following:

    • Provides specific educational materials to families including assisting families in identifying and utilizing natural supports
    • Recruit volunteer sites. Build strong and continuing relationships with volunteer sites and adheres to all the volunteer agency’s volunteer requirements
    • Builds strong and continuing relationships with sources for peer partners e.g schools, universities, youth organizations
    • Builds strong and continuing relationships with each of the funding sources e.g. vocational rehabilitation, DSPD, etc.
    • Conduct outreach activities to community groups to develop referral sources and develop new contacts
    • Provides a continuum of services to families from Pre-Employment Transition Services to Work-Based Learning Experiences
    • Coaches family through the Self-Guided Discovery process
    • Supports families in performing information interviews in the community.
    • Sets up work-based learning experiences as authorized with a focus on supporting both the student and the employer.
    • Completes end of program report in a timely manner and sends to student, family, and VR counselor if applicable.

    Required Skills

    • Proven oral, written, organizational, and record keeping skills
    • Computer skills in Microsoft Office and Excel
    • Ability to analyze and interpret information
    • Knowledge of local community resources

    Required Experience

     

    • 0-2 Years experience working with children and or teens with special needs
    • Equivalent degree or experience
    • Managing a complex flexible schedule
    • CPR Certified

     

    PHYSICAL REQUIREMENTS

    • Ability to move independently or with reasonable accommodation within the facility and community.
    • Applicant must be physically and mentally able to perform all requirements of the job
    • Applicant must pass a background check and DMV check.
