    Job Description

    We are currently in need of an experience Patient Care Coordinator.

    Patient Care Coordinator position is responsible for the coordination and overall delivery of patient care. In this position you would assist with patient assessment and continuity of care before, during and after various procedures. Day-to-day responsibilities require good use of judgment regarding timely and appropriate communication to team members and families.

    RESPONSIBILITIES:

    • Focus on patient priorities with a kind and caring attitude
    • Attend to patient and family care
    • Help patients and health care providers
    • Monitor patient progress and prepare patient for discharge
    • Coordinate specific patient care activities
    • Be the advocate to meet patient care needs
    • Attend to patient scheduling process
    • Verify patient insurance benefits and complete patient registration

    REQUIREMENTS:

    • Medical Assistant Certificate preferred, or equivalent experience
    • Minimum of two years of experience in a medical office setting
    • Experience with computers and word processing
    • High School Diploma or GED
    • Must be bilingual Spanish

    PAY RANGE: 15-$17 DOE
