The Care Coordinator helps to ensure appropriate service delivery of care to our members living with HIV to include member health risk assessments, coordination of transportation and referrals to specialty providers and programs.  Education to members regarding health plan benefits, including expanded or value added benefits, and community resources addressing social determinants of health.  Work with diverse and committed group of individuals with goal of linkage to care and improved health outcomes for our members. Help us lead with integrity and compassion to meet the needs of our members along the continuum of care.

Prefer background and/or interest in working with vulnerable and chronically ill. Especially preferred is experience working with individuals who have serious mental illness, substance use disorder, or both.

• You must be currently enrolled in an accredited college or university with a preferred winter 2021 / spring 2022 graduation date. Rising Juniors and Seniors
• Have an overall cumulative GPA of 3.0 or higher.
• Be motivated and willing to learn with strong communication, organizational, and leadership skills.
• Skilled at working collaboratively, developing creative solutions, and accomplishing team goals.
• Have proficiency with Microsoft Windows software applications (Excel, PowerPoint, Word and Outlook).
• Pursuing degree in sales, marketing, or similar field
• Excellent verbal and written communication skills
• Member focused and customer service orientated
• Compassionate and passionate about health and social issues that impact the community
