Physician (M.D.or D.O)

THIS IS FOR A FUTURE OPENING

STGi is currently seeking qualified Physician (M.D. or D.O.) candidates as contracted medical staff working alongside uniformed Customs and Border Protection (CBP) Agents/Officers in secured federal facilities along the southwest border of the United States. These secured federal facilities are staffed and monitored by CBP personnel and/or contracted security staff 24 hours a day, 7 days a week to assure a safe working environment to include the co-located medical unit. Contracted medical staff work together with CBP personnel to provide health care services for persons of all ages, to include infants, children, adults, and pregnant females.

MISSION

CBPs Office of the Chief Medical Office (OCMO) employs a family medicine model to provide frontline medical care for persons in CBP custody. This model provides a team of medical professionals comprised of physicians, advanced practice providers (e.g., physicians assistants, nurse practitioners), registered nurses, and support staff (e.g., Licensed Practical/Vocational Nurses, Paramedics, Emergency Medical Technicians, Certified Medical/Nursing Assistants). These contracted medical teams provide basic assessment and medical treatment for persons in CBP custody to include emergency care as needed. OCMO is committed to ensuring a system of care that is ethical, responsible, and accountable through surveillance and monitoring activities that are in place.

Major Duties and Responsibilities:

1. Serves as the on-site medical authority within the scope of services provided by DHS/CBP .

2. Clinical supervisory authority over advanced practice providers (APPS) (nurse practitioners and physician’s assistants) as well as other nurses, certified nursing assistants, certified medical assistants, emergency medical technicians, paramedics, laboratory, and pharmacy.

3. Responsible for developing and coordinating all aspects of healthcare delivery at his/her assigned facility.

4. Provides direct care to patients within discipline Scope of Practice and performs functions in accordance with current accepted practice, licensure, certification, credentialing and or granted privileges.

5. Provides age-appropriate screening, treatment, and patient instruction pertinent to the discipline and facility setting for acute and chronic conditions.

6. Assures appropriate diagnostic and therapeutic tests and services are available, utilized and monitored.

7. Acts as initial reviewer for all outside referrals/consults to specialty services not offered at the local facility.

8. Conducts daily rounds on patients under enhanced medical monitoring as needed.

9. May be expected to respond to and or coordinate response to medical emergencies.

10. Seeks guidance from the Medical Director, Regional Physician Supervisor, Pediatric Advisor, or leadership as appropriate.

11. Collaboratively leads the local medical quality management (MQM) committee and oversees local root cause analyses and quality improvement projects/initiatives in collaboration with the local and national MQM committee. Works closely with the Patient Safety Risk Manager for all MQM oversight and management at the local level at assigned medical unit (s).

12. Leads and attends general staff meetings, providing education to all staff on duty.

13. Performs record keeping functions in accordance with program policies and position.

14. Maintains functional proficiency and ease of use utilizing electronic health records as required by, and appropriate to, position.

15. Completes and passes all discipline specific competencies testing, initially and annually.

16. Completes all initial, annual, and ad hoc training as required/assigned.

17. Maintains patient confidentiality, and confidentiality of medical records, in compliance with the Privacy Act and HIPAA regulations in all work activities.

18. Adheres to, and maintains awareness of DHS and CBP Policies, Procedures, Directives, and Operational Memoranda.

19. Leads, attends, and participates in general/medical staff meetings.

20. Provides emergency care to employees or visitors for life, death, loss of sight/limb situations until Emergency Medical Services arrives.

21. Assists with APP Peer review process, reviewing completed chart reviews, providing education and guidance, assisting with addressing any necessary deficiencies noted.Provides education and oversight to APPs who require extended orientation, training, or refresher.    Seeks direction from Supervising Physician as needed.

22. Performs other duties and responsibilities as assigned.

Supervisory Controls:

1. Works under the administrative direction of the Physician Supervisor or in their absence the Deputy Medical Director.

2. Works under the clinical supervision and/or direction of the Physician Supervisor or in their absence the Deputy Medical Director.

3. In this position, the employee will serve as a contractor to the federal government and does not have any federal authority or rights.

Physical Demands:

1. Able to walk, stand, bend, or sit for extended periods.

2. Able to reach to retrieve documents or supplies.

3. Able to carry materials and/or file boxes up to 30 pounds on an infrequent basis or assist patients into/out of a wheelchair.

4. Must be capable of standing on hard surfaces (cement floors) for long periods of time.

5. Must perform the duties in a stressful and often austere environment without physical limitation.

6. Must be able to perform BLS/CPR, as needed.

Required Skills

BASIC REQUIREMENTS Licensure/Certification:

1. Candidates must have a full, current, and unrestricted license or registration as a Doctor of Medicine (M.D.) or as a Doctor of Osteopathy (D.O.) valid in the location in which the work is performed. Licensure must be maintained current, and any required education, trainings, continued learning to maintain licensure must be completed within the required timeline for completion by the licensing authority. A temporary license will not be accepted.

2. Candidate must not have had any professional licensure or certification withheld, suspended, revoked, or denied, and the candidate must not have surrendered or allowed a professional license or certification to have expired or lapsed because of an investigation or disciplinary action.

3. Candidate must not have been convicted of, pled guilty to, pled nolo contender to, or received probation before judgement for any crime other than a minor traffic violation, the record of which has not been expunged.

4. Basic Life Support (BLS) Certification must be from one of the following approved organizations:

   a. American Heart Association (AHA) BLS for health care providers b. American Heart Association Advanced Cardiac Life Support (ACLS), if applicable

   c. American Red Cross CPR/BLS for the professional rescuer d. American Safety and Health Institute (ASHI) CPR and AED for healthcare providers and professional responders

   1. Course components must include Adult, Pediatric, and Infant CPR, Foreign Body Airway Obstruction, and Automatic Defibrillator (AED) proficiency

   2. Applicant/ Employee must be certified upon hire and may also be a certified instructor

   3. BLS/CPR must be renewed prior to expiration date, and always kept in an active status

5. Current board certification in Primary care to include Family Practice and Internal Medicine, Emergency Medicine, or Pediatrics by the American Board of Medical Specialties (ABMS) or American Osteopathic Association (AOA) approved board. All other board certifications will be reviewed on a case-by-case basis.

6. Bilingual in English and Spanish encouraged, but not required.

Required Experience

EDUCATION/TRAINING:

1. Degree: Doctor of Medicine, Doctor of Osteopathic Medicine or equivalent from a school in the United States or Canada. The degree must have been accredited by the Council on Medical Education of the American Medical Association, Association of American Medical Colleges, Liaison Committee on Medical Education, Commission on Osteopathic College Accreditation of the American Osteopathic Association, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
2. Subsequent to obtaining a Doctor of Medicine or Doctor of Osteopathic Medicine degree, a candidate must have had at least 1 year of supervised experience providing direct service in a clinical setting, i.e., a 1-year internship or the first year of a residency program in a hospital or an institution accredited for such training. For purposes of this standard, graduate training programs include only those internship, residency, and fellowship programs that are approved by accrediting bodies recognized within the United States or Canada.

RESIDENCY TRAINED AND BOARD CERTIFIED IS REQUIRED.

Degree from Foreign Medical School: A Doctor of Medicine or equivalent degree from a foreign medical school must provide education and medical knowledge equivalent to accredited schools in the United States. Evidence of equivalency to accredited schools in the United States is demonstrated by permanent certification by the Educational Commission for Foreign Medical Graduates, a fifth pathway certificate for Americans who competed premedical education in the United States and graduate education in a foreign country, or successful completion of the U.S. Medical Licensing Examination.

KNOWLEDGE, SKILLS, AND ABILITIES:

1. Expert knowledge of the current standards of practice in clinical medicine.
2. Ability to serve proficiently as a clinical expert, providing technical assistance and consultation to clinic medical staff.
3. Ability and proficiency to properly use equipment, materials, and supplies in simple diagnostic and treatment procedures.
4. Ability to provide input for congressional inquiries and to effectively communicate with
5. Federal Agencies and other organizations as indicated.
6. Mastery of managerial and administrative practices, to include expertise in routine management of ambulatory clinic functioning and responsibilities.
7. High degree of independence, initiative, clinical judgment, and follow-through on a wide range of sensitive, complex, and clinical issues.
8. Teaching experience acknowledging a level of expertise and capacity for developing and
9. accessing clinical educational programs.
10. Knowledge of issues confronting the healthcare delivery system, including specific problems and concerns of special population groups.
11. Knowledge of program policies and guidelines, and operating procedures relating to healthcare delivery systems.
12. Knowledge of organizations and responsibilities of all levels of government operations.
13. Ability to work in a multi-cultural and multi-lingual environment.
14. Ability to utilize telephonic interpreter/translation services to complete assigned duties if not fluent in a language the patient understands.
15. Ability to adapt to sudden changes in schedules and flexibility in work requirements.
16. Ability to communicate proficiently in English (verbal and written) in order to develop positive rapport with patients, co-workers and other stakeholders.
17. Ability to establish and maintain positive working relationships in a multidisciplinary environment.
18. Ability to navigate in an electronic work environment including electronic health records, web based training and communications.
19. Moderate proficiency in, common Microsoft Office programs, specifically Microsoft Word, Excel, Outlook, and SharePoint.
20. Knowledge of regulations (HIPAA/Privacy Act) regarding the confidentiality of patient medical records and information as well as Personally Identifiable Information (PII).
21. Ability to initiate and maintain compliance with DHS and CBP policies and any overriding federal policies/standards or accreditation standards that may apply.
22. Maintains compliance with standard operating procedures and protocols.
23. Ability to perform record keeping functions in accordance with program policies and the system of records management.
24. Ability to maintain non-involvement in the security aspect of patient care and custody.
25. Assures a safe and clean working environment and assists in the disposal of biologically hazardous waste on a regular basis.
26. Completes all annual training and additional training as required.

28. May be required to serve at various medical units within sector/region or cover rotating on-call coverage.
29. May be required to be available telephonically and/or to report to the medical unit during non-core hours.
