The Home Manager is responsible for overseeing the service delivery and supports to one or more individuals who are supported 24/7 in their own home or apartment. These are exceptional needs individuals who are dually diagnosed with both one or more intellectual disabilities and one or more mental illnesses. The Home Manager will ensure that the individual’s support plan is delivered specific to the person’s needs and that the day to day operation functions with few interruptions and incidents.
At Help at Home, formerly Coastal Home Care, the role of Developmental Disabilities Professional (DDP) is to be the primary person who relates to the individuals we serve who have an intellectual or developmental disability. The DDP gets to know the individuals and their families well because he/she is the one who helps design that person’s support plan.
JOB SUMMARY:
The Developmental Disabilities Professional (DDP) coordinates and oversees the services of individuals with developmental disabilities. Duties include coordinating and monitoring all aspects of service by observing individuals, reviewing data and progress, and revising schedules as needed to meet goals. The DDP is present and participates in the development of all individual support plans and interventions and ensures that the organization is compliant with all state and federal rules and regulations.
