Park Avenue Center is a family owned substance abuse and mental health treatment facility who has been in business in the South Minneapolis area since 1979. We are looking for an individual to join our Medication Team to assist in supervising Medication Technicians and provide education to our clients.
Position Summary:
Serve as first point of contact for clients, chemical dependency technician staff, clinical staff, and other entities regarding clients' self-administration of medications ensuring clients take medications as prescribed while escalating matters to the Supervising RN when necessary
Duties and Responsibilities:
- Provide care coordination of services between clients and on-site medical clinic and with any other resources as needed
- Maintain work schedules for the department, ensuring proper coverage to meet clients' needs
- Complete accurate, detailed, and timely documentation in clients' electronic medical records
- Maintain compliance in all aspects of the position including, but not limited to, completion of the MAR, regular completion of audits, controlled medication counting, and managing client paperwork
- Maintain regular communication with the Supervising RN and counselors regarding client's failure to take medications as prescribed, if medication levels are running low, or if there are any medical concerns
- Inform Supervising RN of any medication discrepancies, any medical concerns the client may have, or any medical changes such as behavioral or mental health changes
- Conduct individual client sessions as requested
- Provide health consultation services to clients and clinical staff within scope of practice
- Participate in weekly multidisciplinary staffing meetings
- Maintain a professional relationship with clients while following a strict code of ethics
Education/Licensure/Certification:
- Must be licensed in good standing as an LPN in the state of Minnesota
- Must obtain and maintain required continuing education units and training
Preferred Knowledge/Skills/Abilities:
- Must demonstrate competence in working with clients with co-occurring disorders; through education, training, and experience
- Ability to work in alignment with company values, goals, policies and procedures
- Knowledge of MN Statute Chapter 245G and licensing and its application to the department and company's policy and procedures; or ability to obtain knowledge after hire
- Ability to establish and maintain effective and cooperative relationships with co-workers
- Ability to maintain excellent interpersonal, verbal, and written communication skills in English
- Ability to establish priorities, manage projects and meet deadlines
- Ability to work both independently and as part of a team
- Proficiency in Microsoft Office programs and electronic medical record software program; or ability to obtain proficiency after training
