    POSITION SUMMARY: 

    Responsible for managing a case load of clients; working with them to obtain appropriate, necessary resources while clients attend treatment and when they transition out of treatment

     ESSENTIAL FUNCTIONS:       

    • Develop and maintain working knowledge of community resources available to clients including health insurance, housing, medication resources, and support groups
    • Facilitate referrals to substance use disorder and/or mental health services as indicated by the client's medical provider, comprehensive assessment, or treatment plan.  Assist with referrals to economic assistance, social services, housing resources, and prenatal care according to the client's needs
    • Provide life skills advocacy and support accessing treatment follow-up, disease management, and education services, including referral and linkages to long-term services and supports as needed
    • Assist in coordination with client's significant other to help in treatment planning whenever possible and meet with families, probation, and social workers to develop transition and contingency plans
    • Assist in coordination with and follow up for medical services as identified in the treatment plan
    • Participate regularly in client staffing to maintain consistent communication with counseling staff regarding clients' treatment services
    • Conduct individualized client sessions weekly or as needed
    • Facilitate a variety of gender-specific groups including, but not limited to, activity and psychoeducational
    • Provide guidance and serve as a resource for questions or concerns within the facility
    • Maintain a positive, professional, and ethical demeanor with clients and within the community while following a strict code of ethics
    • Participate in weekly supervision meetings with supervisor
    • Maintain appropriate charting to communicate clients' transitional plans and documentation of the provision of treatment coordination services in the clients' files while complying with MN Statute Chapter 245G requirements regarding timely paperwork and documentation
    • Work alongside peers to offer and receive support when needed

    KNOWLEDGE/SKILLS/ABILITIES:

    • Must demonstrate competence in screening for and working with clients with co-occurring disorders; through education, training and experience
    • Knowledge of MN Statute Chapter 245G and licensing and its application to the department and company's policy and procedures; or ability to obtain knowledge after hire
    • Must possess strong leadership and management competencies; able to teach, coach, and consult with staff across the organization
    • Ability to work, plan, organize, and evaluate work in alignment with company values, goals, policies and procedures
    • Ability to establish and maintain effective and cooperative relationships with co-workers
    • Ability to establish priorities, manage projects and meet deadlines
    • Ability to maintain excellent interpersonal, verbal, and written communication skills in English; other language(s) preferred
    • Ability to work both independently and as part of a team
    • Proficiency in Microsoft Office programs and electronic medical record; or ability to gain proficiency after training

    EDUCATION/LICENSURE/CERTIFICATION:

    • Must be a Licensed Social Worker (LSW) or have the ability to obtain a Temporary License 
    • Have successfully completed 30 hours of classroom instruction on care coordination for an individual with substance use disorder is strongly preferred; required after hire.
    • A minimum of 2,000 hours of supervised experience working with individuals with substance use disorder is preferred, but not required
