PAC is seeking a SUBSTANCE ABUSE TREATMENT SOCIAL WORKER (SATSW) to work in a positive work environment with a team of supportive professionals. The SATSW is responsible for managing a case load of candidates undergoing mental and chemical health rehabilitation and working to assist clients in obtaining resources needed for successful, lasting recovery.

Essential Functions:

• Cultivate partnerships and connections to community resources, including medical providers and insurance, housing, medication resources, social services, and support groups

• Facilitate substance use and mental health service referrals, as detailed by the client's medical provider, comprehensive assessment, or treatment plan.

• Advocate for clients in treatment follow-up, disease management, and education services, including working with families, probation, and social workers to develop transition and contingency plans

• Treat clients through individual sessions weekly or as needed

• Facilitate client activities and psychoeducational groups

• Document clients' transitional plans and treatment coordination services in a timely manner

Knowledge/Skills/Abilities

• Knowledge of human behavior and performance; learning and motivation; psychological research methods; and the assessment and treatment of behavioral and affective disorders.

• Understanding of group behavior and dynamics, societal trends and influences, ethnicity, and cultures.

• Ability to work, plan, organize, and evaluate work in alignment with company values, goals, policies and procedures

• Exceptional interpersonal, verbal, and written communication skills

• Capacity to work both independently and as part of a team

Required qualifications:

• Minnesota Licensed Social Worker (LSW) or the ability to obtain a Temporary License.

Preferred qualifications:

• Successful completion of 30 hours classroom instruction on care coordination for an individual with substance use disorder is strongly preferred; required after hire.
• 2,000 or more hours of supervised experience working with individuals with substance use disorder is preferred, but not required.
• Knowledge of MN Statute Chapter 245G and licensing preferred.
• Proficiency in Microsoft Office programs and electronic medical record programs.

Job Type: Full-time, benefit eligible, Monday - Friday days with flex/hybrid scheduling possible.

Pay: $48,000.00 - $58,000.00 per year DOQ.  Currently offering a $5,000 Hiring Bonus
