Work involves developing and maintaining long-term contact with clients, client families, and service providers for medical, social, educational, and related service needs. The position works under general supervision with moderate latitude for the use of initiative and independent judgment. Organizational Relationships Reports to: Program Supervisor/Clinic Manager Directs: Not Applicable Other: Has frequent contact with the mental health and/or the individuals with developmental disabilities community and/or mental health clinical staff in order to coordinate and complete assigned responsibilities. May have contact with related internal and external stakeholders. Essential Duties Under the general supervision of a Program Supervisor or Clinic Manager provides on-going case management services, skills training and rehabilitation services for youth and/or adults with mental illness and/or individuals with developmental disabilities. Coordinates and provides related services. Demonstrates a positive role model to youth and/or adults and advocates on their and their family's behalf. Responsible for coordination, planning and implementation of the Recovery Plan. This position requires flexibility for work scheduling purposes and requires on-going training to effectively accomplish position duties and meet performance outcomes. Provides rehabilitative services including general symptom management, community living skills, and employment related skills to increase our client's independence and increase their ability to maintain community tenure. Employee shall maintain compliance with all Joint Commission training requirements relevant to job duties, including but not limited to those dealing with use of restraints and physical holding of individuals receiving services. This position may be assigned to one of the following functional areas: Crisis/Intake, Mental Health. Minimum Requirements: Knowledge, Skills, and Abilities Knowledge of:Knowledge of mental health issues in a metropolitan/urban setting. Ability to communicate with mental health/individuals with developmental disabled clients and their families to promote compliance with treatment plans. Skill/Ability to:Work independently and remain flexible to solve problems and crisis. Communicate effectively both orally and in writing. Meet productivity and performance expectations outlined by EHN. Acceptable Experience and Education Bachelor's degree in a human services/or behavioral sciences related (psychology, sociology, social work) field from an accredited college or university. At least one (1) year of direct casework/case management experience in a community service setting is required. At least one (1) year direct experience working with mentally ill youth/adults in a community service type setting is preferred. No professional license is required. QMHP status is necessary for all caseworker positions and will be facilitated by EHN upon hire. A valid Texas driver's license and good driving record is required along with a copy of a current and valid auto insurance card. Certificates and Licenses Required Must have or obtain by date of hire a Texas driver's license applicable to job responsibilities, with a driving record acceptable to EHN. Must be able to pass a TB, criminal background, and drug screen. Must be able to be credentialed as a QMHP within a timeframe designated by EHN. Possess and maintain recognized current certification, or complete specialized training to ensure up-to-date knowledge of job related skills and technology. Physical Demands While performing the duties of this job, the employee is regularly required to sit and use hands and fingers to handle or feel. The employee frequently is required to reach with hands and arms; stoop, kneel, crouch, or crawl; and talk or hear. The employee is occasionally required to stand, walk, and climb or balance. The employee must frequently lift and/or move up to 25-35 pounds and may manually push/pull a dolly. Specific vision abilities required by this job include close vision, color vision, depth perception, and ability to adjust focus. Must be able to operate a vehicle Work Environment Equipment Used: Computer terminal/data processing software to include case management software; various computer peripheral equipment as required; use of general office equipment as necessary. Work environment also includes visits to the clients residence or respective location in the community.
