    PROGRAM DESCRIPTION AND TRAINING OPPORTUNITIES:

    Continuum Services are an integrated and intensive array of services developed in partnership with youth and families to support them in living safely and productively within their communities. The Continuum Provider is responsible for meeting the treatment needs of each youth and family with clinically appropriate services. The quantity, intensity, and focus of services are tailored to meet the unique needs of each youth and family. A key feature of the Continuum service is continuity of care in the treatment approach.

    • The Continuum Core Service Team remains involved with the youth and family even as their level of care changes within the Continuum.
    • Interventions primarily take place at the location where problems occur, such as at home, in the community, and at school, to support youth and families in learning and practicing new skills.
    • Continuum staff is available to provide services to the youth and family at times and locations which supports their participation.
    • Services are available 7 days a week, including weekends and evenings.
    • Face-to-face crisis support is available 24 hours a day, 7 days a week to support the youth and family in managing crises and solving problems.

    YOUTH SERVED WITHIN A CONTINUUM ARE OFTEN:

    • aggressive
    • engage in behaviors that are harmful towards others
    • have intermittent difficulty maintaining behavioral control
    • self-injurious behaviors
    • struggle with suicidal ideation
    • have a history of suicide attempts
    • engage in behaviors that place themselves at risk, including but not limited to running away or being sexually exploited
    • many youth struggle with depression, anxiety, substance use/abuse, psychosis, or disorganized thinking
    • have had or will need a psychiatric evaluation and ongoing psychopharmacological intervention
    • difficulties with social relationships
    • struggle in school and have learning and/or processing difficulties
    • may engage in fire setting and/or demonstrate problematic sexual behavior.

    YOUTH APPROPRIATE FOR CONTINUUM SERVICES MEET THE FOLLOWING CRITERIA:

    • The youth has a mental illness and/or severe behavioral and/or emotional disturbance.
    • The youth exhibits high risk behavior and/or safety concerns.
    • The family needs the support of intensive community-based services to meet their child’s needs.
    • The youth has a lifelong adult resource(s) to support their treatment.
    • The youth and family are likely to need 24/7 community-based crisis support.
    • The level of risk posed by the youth and family is safely manageable with services provided by a Continuum.
    • The youth and family are willing to participate in intensive family support geared towards helping the youth remain in or return to live in the community. (on average, 6 hours a week)
    • The youth has the ability to spend time in the community with support.
    • Youth are able to attend school in the community.
    • If the youth is in a hospital, DMH Continuing Care Inpatient, CBAT, CIRT, IRTP, or residential school program, he/she may be referred to the Continuum if he/she is ready to participate in supported community-based treatment.

    • 2nd year graduate student in school for: LMHC, LMFT, LICSW
