    Sheltering Arms is seeking a Case Planner to provide case management services to families in our Family Connections Program.

    Essential Functions

    • Evaluate clients to assess risks and develop case plans.
    • Monitors and follows up on individualized treatment programs including child safety.
    • Individualizes and understands the needs of the individual client, the family and its support network in relation to each other.
    • Conduct home visits in which the needs of the family and child safety are assessed.
    • Conduct field visits to identify services for clients and helps families maintain medical, financial, educational and social resources.
    • Complete Required Family Connections Evidence base trainings.
    • Facilitate specialized groups.
    • Identifies appropriate treatment or service providers and facilitates client referrals to internal and external and/ or community linkages.
    • Establishes client goals and assesses a client’s success in meeting those goals; continues, alters and recommends changes in treatment strategy depending upon ongoing client assessment.
    • Advocate on behalf of children and families to obtain the necessary neighborhood based services.
    • Required to complete FASP’s (Family Assessment Service Plan) every 90 day and six months.
    • Adhere to model requirements and fidelity for the Family Connections model.
    • Performs other duties as delegated

  • Qualifications
    • Bachelor’s Degree or Master's degree in related field
    • Knowledge of ACS policies and procedures
    • Previous child care welfare experience preferred
    • Bilingual Spanish.
    • Knowledge of Connections and PROMIS preferred
    • Strong communication skills, verbal and written
    • Microsoft Office skills
