    Sheltering Arms is seeking to hire a Medicaid Service Coordinator for our Developmental Disabilities Services Dept. The Medicaid Service Coordinator (MSC) will be responsible for working with the individual, family and/or advocate to develop, implement, evaluate, monitor and revise the Individualized Service Plan (ISP), based on the individual’s personal goals, needs, interests, and preferences. Assist administration in maintaining quality improvement. Develop a roster of candidates for placement.

    Essential Functions

    • Oversee and coordinate clinical issues.
    • Screen candidates for placement and develop a waiting list.
    • Develop and coordinate the Preliminary Individualized Service Plan (PISP).
    • Develop, oversee implementation and maintenance of the Individualized Service Plan (ISP) based on the individual’s personal goals, choices, and outcomes.
    • Review, monitor and revise the Individualized Service Plan to reflect the changing needs, choices and preferences of the individual, minimally on a semi-annual basis and more often as requested by the individual, family and/or advocate.
    • Forward copies of ISP to all relevant parties in a timely fashion.
    • Ensure that all services described in the ISP are delivered in a timely and appropriate manner. Ensure quality service delivery, as well as, regulatory compliance.
    • Act as liaison between outside service providers and the individual. Encourage integration, individualization, independence, productivity and individual satisfaction in all community activities.
    • Document all contacts and service coordination activities. Prepare monthly service coordinator notes. Ensure documentation is up-to-date, complete, comprehensive, and filed in the individual’s record at all times.
    • Assessment of individual’s needs via face-to-face monthly contact and gathers information from collateral sources
    • Assist with quality improvement chart reviews.
    • Participate in the development of appropriate in-service staff training.
    • Attend conferences, in-services, and informational meetings.
    • Maintains caseload as assigned by supervisor.
    • Record keeping: keeps records up-to-date including case notes, MSC Agreement, Level of Care Eligibility
    • Determination (LCED), medical reports and correspondence with collateral’s, notes of phone contacts.
    • Maintains training requirements per MSC regulations.
    • Other related tasks that may be assigned.

    Qualifications

    • Bachelor’s Degree in Human Services or related field
    • (2) year of experience working with people with a developmental disability
    • Participation in OPWDD CORE training curriculum is required within first 3 months of employment.
    • Knowledge of community resources.
    • Must be familiar with ICF/MR regulations and waiver requirements.
    • Ability to relate and interact positively with residents, staff and other providers of service.
    • Ability to coordinate.
    • Good writing skills.
    • Valid NYS State Drivers License preferred.
