Position Summary The Service Coordinator provides administrative and non-clinical member service support related to the personal care benefit and Managed Long Term Care product-lines and departments. This position is also responsible for the completion of all necessary paperwork and documentation, authorizations, processing MD orders, and securing provider assignments; coordinate documentation retrieval and exchange between departments to ensure smooth workflow. Essential Functions and Responsibilities Responsibilities * Transcribe, format, input, and edit data to create letters, documents, spreadsheets, authorizations and graphic presentations. * Clarify the member's complaint; determine the cause of the problem; select and explain the best solution to solve the problem and expedite correction or adjustment; follow up to ensure resolution. * Create spreadsheets with appropriate data, as requested and assigned; provide others with organized statistical reports or graphs. * Problem-solve nonclinical issues; order and maintain such services as meals on wheels, DVRs, purchasing of medical supplies and non medical essentials. * Maintain monthly contact with members to assure proper care; carry out close tracking of personal care at home; complete care management documentation and perspective Utilization Management to ensure all necessary information and updates are recorded. * Provide triage clinical services and take action to resolve triage emergencies on a case-by-case basis; assist nurses doing high touch clinical cases; assist with reports and requirements. * Alert MLTC of fraudulent activity; assist with KPI policies; perform all steps necessary to complete authorizations and close cases. Occasional home visits required, requires driver's license and reliable vehicle. Educational and Experience Requirements Work Related Experience: * Minimum of 1-2 years Personal Care coordination and Home Care experience. * Experience with member service preferred. * Minimum of 1-2 years related experience required. * Proficiency in Microsoft Office: Excel, Word, Access; Internet. * Bilingual in Spanish, Russian, Mandarin or Cantonese is a plus. * Knowledge of DME, Oxygen and HOYA lifts preferred. * Must have strong analytical and problem solving skills. * Skilled to communicate with all levels of management, internal and external customers. Good communicator and listener. Ability to deal with customers with disabilities. * Skilled in communicating with technical and business constituencies in writing effective business specifications and requirements. Education, License, and Certification Requirements * High School Diploma required.
