    Position Overview

    The Behavioral Licensed Practical Nurse (LPN) at Agape’s Way is responsible for delivering high-quality nursing care and behavioral support to residents. This role focuses on monitoring residents’ behavioral and medical needs, administering medications, supporting individualized care plans, and promoting overall emotional and physical stability within a residential setting. The ideal candidate has experience in behavioral health and a strong passion for providing person-centered care that supports dignity and independence.

    Key Responsibilities

    Clinical Care & Behavioral Monitoring

    • Provide direct nursing care in accordance with individualized service and behavioral care plans

    • Conduct routine health and behavioral assessments, observing changes in condition, mood, or behavior

    • Monitor and document residents’ responses to medications and interventions

    • Support behavioral management plans and utilize therapeutic communication to de-escalate behaviors

    • Communicate significant changes in condition to the Resident Care Director or physician promptly

    Medication Administration & Documentation

    • Administer medications safely and accurately following the seven medication rights

    • Document all medication administration, treatments, and behavioral observations per facility and state guidelines

    • Monitor for medication side effects or adverse reactions and report immediately

    • Maintain accurate Medication Administration Records (MARs) and ensure regulatory compliance

    Collaboration & Communication

    • Collaborate with the Resident Care Director, caregivers, and external providers to ensure continuity of care

    • Participate in care plan meetings and contribute to updates as residents’ needs change

    • Communicate effectively with families, physicians, and interdisciplinary team members

    • Support a respectful, team-oriented environment centered on resident care

    Resident Support & Daily Living

    • Promote resident independence while assisting with activities of daily living as needed

    • Encourage participation in therapeutic and social activities aligned with care goals

    • Provide education related to health, medications, and coping strategies

    • Foster a calm, safe, and positive environment for residents

    Safety & Crisis Intervention

    • Follow facility safety protocols and behavioral intervention procedures

    • Respond promptly to medical or behavioral emergencies with appropriate documentation

    • Identify and report safety risks to leadership

    Documentation & Compliance

    • Maintain accurate nursing and behavioral documentation in compliance with state regulations

    • Ensure confidentiality and HIPAA compliance at all times

    • Participate in audits and quality improvement initiatives as needed

    Professional Development

    • Maintain current Florida LPN licensure, CPR, and First Aid certifications

    • Attend required trainings, in-services, and continuing education

    • Stay informed on best practices in behavioral health and senior care

    Required Qualifications

    • Active Licensed Practical Nurse (LPN) license in the state of Florida

    • Minimum of 1 year of experience in behavioral health, assisted living, or long-term care

    • Strong knowledge of behavioral health principles and medication administration

    • Excellent communication and interpersonal skills

    • Ability to manage behavioral challenges calmly and professionally

    • Strong attention to detail, organization, and teamwork

    • Ability to lift or assist up to 50 pounds as needed
