  • Responsibilities

    180 Health Partners works with health plans and physicians to identify, engage, and support individuals facing substance exposed living make positive changes in their lives.

    Enrollment Specialists identify and enroll individuals facing substance exposed living and help them understand the benefits of participation in the 180 programs. The Enrollment Specialist will work on the phone and Internet and may travel to meet and enroll an individual in person. This role may be an individual’s first opportunity to share needs or concerns and, as such, the Enrollment Specialist will participate in crisis training and acquire a basic understanding of resources available through 180’s Resource Advocacy team. The Enrollment Specialist will work with referral sources in instances where an individual is difficult to reach or to confirm contact information.

    The Enrollment Specialist operates with minimal supervision, elevating unusual situations to management as required. There will be regular meetings with your manager. A successful Enrollment Specialist is someone who does not give up easily, is self-motivated, conscientious, empathetic, and a creative problem solver. ‪

    Responsibilities

    Serves as an initial outreach from the company, primarily through tele-engagement

    Serves as liaison to facilitate contact with the clinical team at time of enrollment

    Provides follow up for the clinical team when patients are unable to be reached following enrollment

    Quickly notifies the Resource Advocacy team or clinical team when the Mother has an urgent need.

    Completes documentation for client contacts and care plan based on 180 policies.

    Responsible for achieving set goals/Key Performance Indicators (KPI's).

    Promotes responsible and ethical stewardship of 180 resources.

    Daytime travel 25% - 50%.

    Maintains excellent punctuality and attendance during work hours.

    Other duties as assigned.

    Qualifications

    Required:

    2+ years of experience working with tele-engagement in health, social or community services.

    Working knowledge of social and health issues.

    Ability to problem solve in a proactive, creative manner while meeting deadlines.

    Ability to work independently within a virtual operating environment and as part of a team.

    Excellent communication skills.

    Reliable transportation and valid driver’s license.

    Computer proficiency; must be able to type and navigate a Cloud-based environment and designated teleconferencing application.

    Experience with Microsoft Office – Word, Excel, Outlook, etc.

    Demonstrates the philosophy of 180 through values and practices.

    Preferred:

    Experience working with substance exposed individuals

    Multi-lingual capabilities
