    Peer Recovery Specialist (CPRS)

If so, and you are a Certified Peer Recovery Specialist and have successfully participated in a recovery program, 180 Health Partners wants you to join our team!Peer Advocates serve as a role-model for individuals facing substance exposed living and are in varying phases of recovery. In this role, you would follow 180 Health Partners’ guidelines and trauma-informed philosophy in planning care, assessing client needs, and completing screenings. You also would refer clients to resources as needed, and coordinate with the 180 Health Partners’ team, primary care providers, managed care organizations, and community agencies to provide quality care for our clients. You will enjoy the flexibility and autonomy to work from home in a field-based, mobile environment while providing services in provider offices, home, community, and telehealth settings across a region. Our teams address substance exposed living for adults with an emphasis on women’s health and opiate exposed pregnancy.     Responsibilities

    Identifies individual client needs and documents intervention based on relationship and assessment.

    Provides support to clients through variety of settings and assists with support groups, as needed.

    Contributes to case conferences to ensure team approach is delivered for each client.

    Completes documentation for client contacts, touches, cancellations and care plan based on 180 policies.

    Communicates professionally with clients, families, physicians, community agencies, agency leadership and office staff.

    Follows chain of command in communicating concerns or recommendations.

    Responsible for achieving engagement targets in order to realize quality outcomes.

    Represents and promotes 180 through participation and interaction with community, professional organizations and community outreach efforts.

    Participates in timely conflict resolution and treats people fairly and with respect.

    Participates in problem solving and performance improvement processes.

    Promotes responsible and ethical stewardship of 180 resources.

    Daytime travel 25% - 50%.

    Maintains excellent punctuality and attendance during work hours.

    Qualifications

    Required:

    Must be certified as a Peer Recovery Specialist.

    Demonstrates the philosophy of 180 through values and practices.

    Minimum high school diploma or equivalent.

    Reliable transportation and valid driver’s license.

    Proficiency with using Cloud-based systems, EMRs, and teleconferencing applications

    Preferred:

    Certified Peer Recovery Specialist for more than one year

    Experience using Salesforce and TherapyNotes

    Previous experience working from home and organizing daily and weekly tasks

    A background in maternal/substance use populations

    Familiarity with Medicaid/Medicare/Commercial Insurance practices
