    WHAT IS A NAVIGATION SERVICES OUTREACH SPECIALIST/NAVIGATOR?

    Under the direction of the Navigation Services Supervisor, the NAVIGATION SERVICES OUTREACH SPECIALIST/NAVIGATOR provides outreach to potential program candidates, completes accurate client intake for programs and schedules client appointments for Health-Housing Navigators III. Carries a small case load of Clients as described below.

    A Navigator serves as a single access point for anyone in need of services to improve their health/housing and well-being. A Navigator provides care navigation to address the health, housing and social needs of the whole person and works to connect, empower, educate, and advocate for clients with health needs, specifically those experiencing barriers accessing care and/or managing chronic conditions in the uninsured /underinsured populations. A Navigator is passionate about healthy living and has education and experience in the health and wellness and/or housing fields. A Navigator is responsible for completing in-depth holistic health assessments and establishing care plans to address the needs of each client. This care plan includes continual follow ups, addressing the progress of each client, and creating a foundation of support through connection to community partners that provide the applicable services. By assisting our clients with their long-term needs through education, advocacy, and client empowerment, 2-1-1 San Diego is able to address risk factors and social determinants of health to help clients achieve a better quality of life and improve health outcomes.

    ESSENTIAL RESULTS

    Provide Outreach and on-going Navigation Services to 2-1-1 Clients

    • Serve as the Outreach Specialist for the Health Homes Program (HHP) in partnership with Blue Shield Promise Health Plan and Child Welfare Services (CWS) program.
    • Completes daily outreach calls to eligible clients for the HHP and CWS program. Provide empathic, responsive and relevant interactions with potential clients.
    • Complete all intake information including receiving referrals and information calls and schedule First case management appointment for Health-Housing Navigators III.
    • Provide Care Coordination and Housing Navigation services to high acuity cases and provide support and leadership to Navigators on complex cases.
    • Provides application assistance for public and community services.
    • Collaborates with team, direct supervisor, and clinical supervisor to assist with treatment planning, problem solving, and advocacy for clients with complicated housing needs who are lacking resources.
    • Identifies and creates case plans for clients and navigates work with clients based on 14 different social determinants of health.
    • Participates in weekly meetings with the Navigation Team and other related trainings.
    • Ensures successful service delivery by continuously monitoring and improving personal productivity and efficiency, call monitoring scores, and client satisfaction scores.
    • Seeks coaching and feedback opportunities to identify how results can be improved.
    • Participates in continuing education activities & training in order to remain knowledgeable regarding applicable changes in housing services delivery system and eligibility requirements.
    • Accurately documents all client interactions in 2-1-1 and partner systems and explores creative and culturally competent ways to gather necessary information from clients.
    • Consistently documents unmet needs to assist in the identification of gaps in services that can be addressed through 2-1-1 data reporting.

    • Collaborates with and serves as a resource to community organizations and 2-1-1 staff in order to share best practices and other information.
    • Provides health-related education to other staff members and trains new team members as needed.
    • CONTINUE TO PROVIDE SUPPORT WITH THE 211 CALL CENTER ON A REGULAR BASIS.

    WHAT YOU BRING

    • Bachelor's Degree in the fields of social services, public health or health care. Continued education beyond High School combined with relevant experience may be substituted.
    • At least one year of experience in similar Outreach/Intake Specialist role. Case management experience a plus.
    • Must possess excellent communication and relationship building skills, ability to work with various cultural and socio-economic groups, including people in crisis, transition and homelessness.
    • Previous call center or heavy telephone work strongly desired.
    • Demonstrated ability to work with vulnerable populations including people experiencing crisis
    • Motivational interviewing and strengths-based approach skills strongly desired.
    • Ability to multi-task in a hectic environment, with prompt attention to caller's needs.
    • Ability to actively listen and respond to people in need.
    • Ability to interact effectively with persons experiencing a problem situation or crisis to assess, de-escalate, and provide appropriate coordination to services.
    • Demonstrated critical thinking and analysis skills
    • Strong capability in business software programs including MS Office, Outlook and online applications. PLEASE NOTE THAT COMPUTER SKILLS TEST MAY BE REQUIRED.
    • Bilingual preferred
