Position Overview: We are seeking a highly organized and dedicated individual to serve as our 245D Designated Coordinator. This leadership role is essential to ensuring compliance with Minnesota’s 245D regulations and delivering high-quality services to the individuals we support. The Designated Coordinator will oversee key operational tasks, including client intake, service coordination, training, reporting, and file maintenance. This role supports the safety, well-being, and success of both clients and staff while promoting a person-centered, compliant, and efficient environment.
Key Responsibilities:
Regulatory Compliance: Ensure ongoing compliance with all 245D licensing requirements, including maintaining documentation, signatures, and service delivery standards.
File Maintenance: Keep all required documentation, client files, and records up to date and audit-ready.
Intake & Planning: Facilitate client intakes and collaborate with staff to create individualized, person-centered support plans.
Reporting: Prepare and submit required quarterly and annual reports that support client outcomes and service planning.
Problem-Solving: Proactively address issues as they arise, utilizing sound judgment and creative thinking.
Client Coordination: Monitor client symptoms, attend appointments as needed, and ensure clear communication between team members to maintain client safety and quality of care.
Staff Supervision & Training: Orient and train direct care staff on client needs and individualized services; provide ongoing supervision and support.
Qualifications:
Bachelor’s degree in a human services-related field or four years of full-time experience in the human services field.
At least two years of direct experience working with individuals with disabilities and/or mental illness.
Minimum of two years’ supervisory experience in human services.
Strong knowledge of 245D licensing and service coordination.
Excellent communication, multitasking, and problem-solving skills.
