Job Summary*

We are looking for a DSP Lead to join our team! You will be directly working with clients with disabilities, following a person centered support plan in the homes of the clients. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. If you are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.

Responsibilities

Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor

Build relationships with clients(core members) by sharing life with them and supporting their daily goals

Pass medications

Provide daily transportation for adults we serve to and from work, day service, or community outings.

Support our core members in spirituality, mental wellness, emotional support, and hygiene

Be a positive support in the community as both a DSP and co-worker

Qualifications

High School Diploma or GED

One-year prior professional experience

Driver’s license required

CPR certification will be provided
