Job Description A&A Health Services is looking for an LVN to join our team in our San Pablo community. We provide transitional housing and resident-centered care/wraparound services for clients suffering from mental health, addiction, and/or dual diagnosis. The ideal candidate wants to help break the cycle of clients moving from crisis centers to housing instability and back to crisis centers. Essential Functions of the Job - Oversees and provides direct Resident Care and provides supervision to Mental Health Workers, and Med Techs. - Administers medication and treatments to residents and coordinates and participates in counting medications and giving reports at the beginning and end of each shift. - Collects data relevant to the care of our clients and develops and implements Resident Care Plans and documents the progress of clients toward their recovery plan goals - Work with crisis management and intervention. - Assist in the welcoming, admissions, and discharge processes. - Must possess proper licenses to legally work in CA.
