    QUALIFICATIONS FOR OCCUPATIONAL THERAPIST - OT HOME HEALTH:

    • Valid California Occupational Therapist (OT) license
    • CPR Certification
    • Reliable transportation and valid driver’s license with proof of insurance required.

    RESPONSIBILITIES AND DUTIES:

    • Complete discharge planning by consulting with physicians, nurses, social workers, and other health care workers.
    • Document patient care services via charting in a timely manner.
    • Promote maximum independence by selecting and constructing therapies according to the individual's physical capacity and interest
    • Evaluate results of occupational therapy by observing, noting, and evaluating patient's progress; recommend and implement adjustments and modifications
    • Implement physician-ordered occupational therapies for self-care activities
    • Help the patient develop or regain physical or mental function, or adjust to their disability
