    JOB SUMMARY / JOB RELATIONSHIPS : Identify and assess psycho-social and cultural needs of the resident/patient population, providing psychological counseling and/or referral. To provide direct social work intervention to patients, residents, and families to facilitate their understanding and resolution of problems related to physical illness, environmental stressors, interpersonal conflicts, return to home, and relocation/placement as relative to the utilization of the hospital, walk-in clinic and long term care services. Acts as a resource person to ancillary staff for problem solving difficult situations with advanced assessment and intervention skills. Responsible to the Director of Social Services and Director of Quality & Risk Management. Daily interaction with co-workers and other hospital personnel, medical staff, patients/residents, and visitors.

    EDUCATION : Bachelor degree in Social Work; or Bachelors degree in Psychology with State of Michigan Social Work License.

    LICENSE / CERTIFICATION : State of Michigan Social Work License in good standing. CPR preferred.

    TRAINING / EXPERIENCE / SKILLS : Minimum of 1–2 years of experience; 3–5 years preferred. Excellent communication skills – verbal, written, in person, and via phone. Interpersonal skills a must with co-workers and other hospital personnel, medical staff, patients/residents, and visitors. Ability to competently use office equipment, including: computer, fax, copier, multi-line telephone. Ability to organize, document, and maintain records. Must be highly motivated. Ability to work independently with little or no supervision. Must be an effective team member.

    DUTIES / RESPONSIBILITIES : Maintains confidentiality of pertinent patient/resident care information to assure patient/resident rights are protected. Coordinates initial interview with patients, residents, and/or families, to obtain social history information. Shares and interprets information with appropriate staff, including written summaries for the medical record. Utilizes excellent customer service skills at all times. Provides direct social work interventions to residents, patients, or family as appropriate throughout acute care, skilled care, or long term care stays. Places emphasis on the needs of the patient and family as related to their age and functional abilities. Provides information and assistance to residents, patients, and families as to Medicare, Medicaid, private insurance, and financial assistance programs available to facilitate their needs. Assists residents and families through the admission process to long term care through interviewing, coordinating, and reviewing physical and psychological assessments, financial background information assuring that the admission criteria is met. Assist, organize and implement arrangements for discharge planning for acute and skilled care Hospital patients, to various health care institutions or providers as necessary to maintain their quality of care and life. Educate patients/families of community agencies and services as appropriate to include volunteer organizations for respite care and care-giver support. Document social work activities in the resident's or patient's medical record and on departmental forms.

    Maintain established hospital and long term care policies and procedures, performance improvement program, safety, environment of care, management of information, and infection control standards. Complies with federal and state law and accrediting and licensing agencies at all times, including but not limited to: JCAHO, Critical Access, and State of Michigan standards. Participate in long term care and skilled care multi-disciplinary care team conferences to complete contingency plans, short and long term goals, and discharge planning needs to include referrals to community resources as appropriate. Review and investigate resident and family complaints. Assist in facilitating conflict resolution. Communicate complaints and solutions to LTC Director of Nursing and Social Services Director. Maintain contact with residents/patients/families involving them with non-medical information progress reports as necessary. Coordinate and initiate social services agency (ies) referrals for MR/MI screening and OBRA completion as necessary. In the case of a required Level II Screening, coordinate referral with appropriate community mental health agency. Maintain beauty/barber schedules, with proper system for reimbursement for services rendered. Attend and participate in social work in-services, training, meetings, conferences, and workshops. Coordinate and schedule television and telephone services for residents as requested. Serve on long term care, hospital, or departmental committees as assigned. Maintain records and logs. Other duties as assigned.

    GENERAL RESPONSIBILITIES : Responsible for acting in conformance with all Hospital and Departmental policies and procedures including those relating to Human Resources, Service Excellence, Code of Conduct, Ethics, Health and Safety, Corporate Compliance, Patient Rights, Confidentiality, and HIPAA. Responsible for acting in conformance with Joint Commission Standards. Attends department and Hospital meetings. Participates in department and Hospital activities and in-services. Participates in Quality Improvement activities.
