DESCRIPTION

Organizational Objective:

To provide comprehensive social work services to meet the needs of the gero-psychiatric population during admission, treatment and discharge phases; coordinate and implement recovery and community services programs for the facility

Purpose of Position:

Plan, organize and implement a full range of clinical social work services to patients and their support systems in specific admission and/or long term care program areas; complete ongoing assessments; provide treatment to patients for recovery based on a sound therapeutic approach, and to coordinate discharge. Serve as active member of the interdisciplinary recovery team. Provide education and support services to patient's primary support system. Provide clinical training for social work staff and interns. This position will also assist with program development and performance improvement as needed

Considerable knowledge of human behavior and development, social systems, social work principles and practices, and social assessment methodology, case management and therapeutic counseling. Ability to provide social services and assistance to patients; coordinate agency and community resources; communicate orally and in writing all assessments and interventions with patients, families and external agencies; and to work effectively to solve problems. Knowledge of psychiatric recovery. Ability to write and implement treatment plans. Familiarity with computer use

POSITION REQUIREMENTS

Qualifications:

Education, Experience, Licensure, Certification required for entry into position:

Graduation from an accredited school or university, MSW required, LCSW preferred. Require experience or knowledge in clinical social work, therapeutic counseling, conducting psychosocial assessments and discharge planning. Prefer experience working in inpatient psychiatric facility with mentally ill older adult patients. Prefer knowledge and experience in implementing psychiatric recovery principles. Prefer experience in a treatment team setting
