    The Licensed Clinical Social Worker (LCSW) functions as a core member of the Integrated Care Team within ACI Federal.  The LCSW is responsible for coordinating and supporting mental health and behavioral health care with the practice. This position should have experience working with patients who have co-occurring mental health, substance abuse, and physical health problems. As well as skill and expertise in conducting group and individual psychoeducational classes and brief, short-term therapy strategies that will help strengthen the patient's overall physical and mental health. Additional job functions will include:

    • Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health clinicians.
    • Comprehensive understanding of the interrelationship between medical and psychological systems and their impact on health.
    • Working knowledge of Clinic/ Hospital policies and procedures.
    • Knowledge and experience with evidence-based practices, including trauma informed care framework.
    • Strong clerical/administrative skills including personal computer skills (Microsoft Office software)
    • Ability to make routine decisions and determine proper action based on experience within the limits of policy and procedures.
    • Ability to work effectively with people of diverse cultures, ages and economic backgrounds in a cultural competent and sensitive manner.

    EDUCATION/TRAINING/LICENSE & CERTIFICATION:

    • Master's Degree in Social Work or related field from accredited University. 
    • Licensed Clinical Social Worker (LCSW) or License Marriage and Family Therapist (LMFT) from
    • Michigan.
    • Training provided for a battery of assessments include but not limited to MMPI2rf, MMPI-3, SASSI 4 , and Beck SI.
    • BLS required or obtained within 6 months of hire.
    • LCSW or LMFT in good standing in State of Michigan with minimum two years post-licensure.
    • Paid professional membership and licensure

    ESSENTIAL FUNCTIONS & DUTIES:

    LICENSED CLINICAL SOCIAL WORKER

    • Productivity expectation is an average of 8 – 10 consultants per clinical day.

    • Individual, couples, and/or family consultation with patients including;

      • Functional and strength based-assessment and diagnosis
      • Psychoeducation for patients and their support systems.
      • Medication adherence counseling and disease self-management counseling.
      • Motivational interviewing to develop behavioral strategies aimed as symptom reduction.
      • Brief problem solving cognitive intervention aimed at modifying negative thinking and promoting self-efficacy.
      • Self-Care plan development and skills training to facilitate disease self-management, improved coping, distress tolerance, stress reduction, and relaxation.
      • Substance use/abuse evaluation, identification of maladaptive coping strategies, and development of harm reduction strategies.
    • Consultation with PCP's and or the DBH to enhance understanding of the patients, provide decision support for treatment planning and assist in the implementation and monitoring of biopsychosocial treatment plans.

    PSYCHOTHERAPY

    • Productivity expectation is an average of 5 psychotherapy sessions per clinical day.
    • Formulate, develop and implement a comprehensive psychosocial treatment plan utilizing appropriate clinical treatments and intervention.
    • Provide quality psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment and/or life situation.
    • Case collaboration with supervisor.
    • Process referrals to psychiatric providers, or other practitioners for psychiatric testing or other mental health and behavioral health care needs.

    COORDINATION OF CARE

    • Provide consultation to and coordinate care of patients with primary care staff.  Identify, refer, and advocate for patients needing specialty behavioral health service, and other services as needed.

    CRISIS INTERVENTION

    • Be immediately available to the family practice staff during working hours in the event of a psychiatric emergency.
    • Complete a comprehensive risk assessment and additional assessments as necessary
    • Provide brief therapy services.
    • Formulate safety plans.

    CHARTING

    • Complete all charting as needed and required; including assessments, treatment plans, progress notes, quarterly reviews, discharge summaries and billing.
    • Maintain confidentiality in compliance with HIPPA and RHC regulations.
    • Pursue ongoing continuing education to stay current/up to date with best practices standards and licensure requirements.
    • Maintain a visible presence in the family practice and attend requested meetings.

    GENERAL RESPONSIBILITY AS REQUIRED FOR POSITION

    • Responsible for strict conformance with all applicable accreditation standards (e.g. Hospital, DNV, state, federal) health and safety rules, policies and procedures.
    • Attends department and Hospital meetings.
    • Participates in department and Hospital activities and in-services.
    • Participates in Quality Improvement activities.
    • Demonstrates accountability to accept responsibility for personal/professional growth and required certification/licenses.
