SUMMARY
Under general direction, is responsible for providing evaluation, diagnosis and therapeutic intervention in accordance with agency policies, regulations and requirements of applicable regulatory agencies and standards of profession. ESSENTIAL DUTIES AND RESPONSIBILITIES
The duties and responsibilities of the Physical Therapist - Various will include, but are not limited to the following:
· Provides physical therapy evaluation on individuals annually in areas of muscle tone and/or strength, range of motion, reflex patterns, gross motor capacity, posture and joint alignment, patterns of locomotion and/or ambulation, activities of daily living, including transfers and wheelchair management, need for adaptive equipment and recommendations for changes in present equipment.
· Develops a treatment plan and goal based on assessment.
· Selects educational, recreational and social activities in order to aid consumers in adjusting to their disability and teaches adaptive skills necessary to perform daily living and/or vocational tasks.
· Integrates and provides therapeutic services in or home of person served through consultation with parent/guardian and staff.
· Documents therapy activities and results in patients’ records in accordance with practices’ policy and procedures, assists in overall evaluation and rehabilitation of consumers’ medical/psychiatric condition
· Records progress on all active cases in therapy, monitors therapy status, and makes appropriate recommendations as changes occur.
· Refers patients to other clinical services if such referral is felt appropriate.
· Follows all regulations regarding service provision and billing.
· Performs other related duties as requested.
QUALIFICATIONS

· Ability to read, write, speak and understand English
· Good interpersonal skills are required in order to interact effectively with employees, co- workers, consumers, their families, other programs, outside contacts, etc.
· Due to sensitive nature of the position, must be able to maintain the strictest confidentiality.
· Ability to work independently and handle multiple tasks.
· Good organizational skills
· Good oral and written communications skills
· Analytical ability necessary to evaluate consumer’s condition and progress and select appropriate therapy techniques.
EDUCATION and/or EXPERIENCE
· Master’s in Physical Therapy.
· Two years of experience working with individuals with intellectual and developmental disabilities. · IPSIDD approval preferred.
LICENSE/CERTIFICATION
Current NY State license in Physical Therapy
