Responsibilities:
Develop care plans using a variety of treatment techniques which provide a wide range of physical therapy treatments, including exercises, manual therapy, and modalities such as ultrasound or electrical stimulation.
Home Modification and Safety Recommendations: Evaluate the patient's home environment for potential risks and provide recommendations for modifications or equipment to guarantee a safe and accessible living space. Teach patients' relatives how to use assistive equipment safely and how to prevent falls.
Maintain the records of patients and effectively document
Educate and Inform patients, their families, and healthcare professionals about their diseases, treatments, and at-home fitness regimens. Give advice on body mechanics, how to avoid injuries, and ways to control pain and speed up healing.
Continually evaluate patients' progress, make necessary adjustments to treatment plans, and decide on suitable discharge plans. To ensure a seamless transfer from homecare to alternative rehabilitation settings or community-based programs, work with other healthcare providers and local resources.
Motivate patients during treatment in order to help them function optimally.
Follow all healthcare regulations, professional standards, and facility policies. Stay updated on industry best practices, evidence-based interventions, and advancements in physical therapy.
Collaborate effectively with other healthcare professionals such as nurses, doctors, PTA occupational therapists, and speech therapists.
Use a variety of physical therapy methods, exercises, and strategies to meet the unique needs and objectives of each patient. This could involve assistive technology use, manual therapy, gait and balance training, pain management methods, and therapeutic exercises.
Conduct in-depth assessments of patients in their homes to determine their physical health, functional abilities, and safety issues. Collaborate with other healthcare providers involved in the patient's care while compiling pertinent medical histories.
Qualifications:
A Doctor of Physical Therapy (DPT) degree
Holds a active NJ license in PT
Valid CPR Certification
Prior experience working as a physical therapist, preferably with experience in homecare or community-based settings.
Skills:
Strong interpersonal and communication skills
Strong leadership and supervisory skills
Ability to work independently and as part of a team.
Applicants must possess basic computer operating skills.
Familiarity with electronic medical record (EMR) systems and documentation practices.
Ability to manage patients with different types of personalities.
