Duties:

Our homecare Assessment Nurses will provide initial and continuing in home care assessments for new and existing home care clients, and provide other duties as listed but not limited to the following:

Provide initial and continuing supervisory assessments for home care clients.

Create detailed care plans for home care clients based on client needs and desires.

Document and chart updates, changes to care plans, changes in client needs, and client progress.

Communicate with office staff on immediate changes, health emergencies or client concerns. Also be willing to communicate with client family or case managers as requested.

Be willing to uphold, promote, and enforce all health and safety regulations.

Qualifications:

· Minimum - Associates of Nursing Licensed in Georgia

· COVID Vaccinated providing proof as required by the Executive Order placed on 9/9/2021 for all Healthcare Workers

· Current CPR Certification

· Current Driver’s License

· Fingerprint Background Report

· Able to Pass Drug Screening

· Home Care experience a plus but not required
