SERVICE OVERVIEW
Description
The Dentist will be responsible for providing dental screenings to all youth entering placement at a state operated secure care facility within ten days of admission (excluding weekends and holidays) with a subsequent six month follow up. This Contract is entered into to provide these dental services for youth in with a state program (will be shared with you during the interview process)
TARGET POPULATION
Age, Gender and Service Capacity:
Under the terms of this Contract, the CONTRACTOR agrees to provide dental services for males and females housed in the State-run secure care facilities which will be identified in the interview.
SERVICE COMPONENTS
This Contract has been developed to provide dental services for youth as requested by, and in consultation with, the Superintendent or their designee.
A dental hygienist and/or dental assistant, contracted by the DEPARTMENT, may be available to assist the CONTRACTOR with preparing youth for treatment, sterilizing instruments, and other activities as needed. The CONTRACTOR and the dental hygienist/dental assistant will mutually agree upon a work schedule to maximize dental services. The CONTRACTOR will be responsible for supervising the dental hygienist/dental assistant’s hours and responsibilities.
The CONTRACTOR agrees to perform and provide services which shall consist of, but not be limited to, the following:
Hours of Dental Services per Week:
The CONTRACTOR shall provide dental services within an agreed upon scheduled block time to be determined with the Ferris School Superintendent or designee. Services shall be provided on-site at a designated school (shared in the interview). Youth identified in in the Contract will be made available to receive dental services at the School location.
Dental Care:
The dental care for each youth shall consist of, but not be limited to, the following:
Routine dental services including cleaning, extractions, fillings, and dental hygiene.
Dental sealants and topical fluorides will be provided as deemed necessary by the dentist.
Dental screening examinations:
Dental screening shall include charting decayed, missing and filled teeth and taking a dental history for the purpose of identification of problems.
Youth who are in facility residence in excess of six (6) months, upon referral to the dentist, will receive an additional dental examination to consist of notation of external and internal structure of the mouth, abnormal function, diseases of the mucous membrane and jaws and diseases of the teeth and supporting structures. The examination will consist of diagnostic x-rays, testing of pulp, if applicable, and susceptibility of cavities.
Prostheses:
The CONTRACTOR shall prescribe prostheses when the health of the youth would otherwise be adversely affected. This includes cosmetic work when the DEPARTMENT’S facility Superintendent determines that such service will contribute to rehabilitation.
Meetings, Consultations and Expertise:
The CONTRACTOR shall be available for meetings, consultations and provision of expertise to the DEPARTMENT and DYRS in the delivery of health care services to youth residing in its institutions.
