POSITION: Nurse Practitioner for House Call Practice
POSITION PURPOSE: Visiting patients at home for the purpose of delivering primary care medicine in the patient's home (private residence, assisted/independent living, group homes and memory units). A strong background in geriatrics/internal medicine is required, computer literacy and ability to work independently.
GENERAL DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
- Provide primary care to elderly patients at their places of residence
- Perform examinations, order diagnostic test
- Prescribe medication and home health treatment
- Teach and advise patients and their health caregivers about their health issues and how to manage their illness
- Monitor treatments and keep detailed records of patient care
- Consult with physician as needed to coordinate patient care
- Establish and maintain a presence on partner CCRC campuses
- Attend presentations introducing services to prospective patients
EXPERIENCE / EDUCATION / CERTIFICATION REQUIREMENTS:
- Adult/geriatric NP or family medicine NP
- Experience caring for older adults
- Must be willing to travel within the Greater Las Vegas area
- Must have good computer skills
- Must have excellent communication skills
- Ability to work independently
- Must be organized
- Must have great relationship building skills

PHYSICAL REQUIREMENTS:
- Must be able to drive and carry a backpack with basic medical equipment and a computer
- Must be a good team player; able to work with doctors and colleagues
- Must have solid people relationship skills
- Must be able to relate well with patients
- Limited bending turning, kneeling and stooping, lifting, sitting and standing
- Repetitive motion required including computer entry
