Job Description:

• Job Type: PRN
• EMR: WellSky (Kinnser)
• Home Health experience (nice to have)
• Conducting comprehensive health and psychosocial evaluations of patients
• Monitoring patients' conditions and promoting sound preventive practices
• Coordinating services and providing patient education and training activities
• Performing admission, transfer, re-certification, resumption of care, and discharge OASIS for home care patients
• Collaborating with attending physicians to alter Patient Care Plans as needed
• Coordinating patient services and ensuring continuity of care with other healthcare professionals
• Submitting timely clinical notes, progress notes, and other required documentation
• Participating in case conferences and contributing to the patient's discharge planning process
• Engaging in professional development activities, including in-service education, workshops, conferences, and research

Pay Range:

• $45.00 - $110.00 per hour

Work Location Seminole, FL (On the road)

Weekly schedule:

• Choose your own hours
• Monday to Friday

Requirements:

• FL RN License
• Current CPR certification
• One or more years of experience in a community/home health agency setting
• AHCA Level II Background Screening
• WellSky (formerly Kinnser) software knowledge preferred
• 1+ year in clinical setting
