Position Summary
The Case Management Support Specialist is responsible for supporting the delivery of quality member care to members of the EDWP/SOURCE program. This position applies skills in monitoring records and charts and in maintaining effective working relationships with others. This position combines the activities of direct and indirect care to members. The position also interacts with Personal Support Services Providers and other stakeholders.
- Support the case management department with any outstanding caseloads and management of members.
- Management of new members pending state approval by ensuring member is kept well-informed that Ace exists as their Case management company and document same.
- Support case managers with any member contact that is considered delinquent and on risk for suspension due to unable to make contact.
- Assist members with expired Medicaid reinstate their Medicaid status and reenter the EDWP.
- Monitor members who are at risk for increased income who could lose their EDWP inedibility due to increased income or changes in their Medicaid status that would deem them ineligible for EDWP.
- Make periodic drive by visits to contact members who cannot be reached via phone and also to investigate a matter that requires in-person contact.
- Support the department in the resolution of member issues to ensure adequate communication with members and all stakeholders.
- Ensure adequate communication between CM department, compliance, and all other departments as it relates to cost sheet billing report discrepancies, change in member status, change in member services etc.
- Assist the Case Management Department and its delegates in the day-to-day operations of the departments needed.
- Assignment of projects that will be undertaken by the department from time to time to ensure compliance within the department.
- Provide back-end support and customer service to patients, providers and all stakeholders in a courteous and professional manner in accordance with performance standards.
- Respond to requests from members, family members, physicians and staff members within a timely and efficient manner.
- Attend IDT meetings as needed to ensure adequate and correct information is passed on to the Medical Director and teammates.
- Assist in provider meeting bi-Annually
- Responsible for taking on additional tasks within the department as needed.
